Alabama community health workers (CHWs) help people understand health information, reach services and follow through on care. Training and employer requirements depend on the program. For organizations, the key distinctions are workforce preparation, Alabama Coordinated Health Network (ACHN) care management, the funding agreement and the evidence needed to show participant progress.

What does a community health worker do in Alabama?
A CHW connects community knowledge with practical support. The role may involve explaining health information, helping someone contact a resource, identifying barriers to care, supporting self-management or following up after a referral. Alabama public-health training resources describe communication, relationship building, service coordination, advocacy, outreach and professional conduct as core areas of preparation. [6]
For a participant, the useful result is often concrete: understanding the next step, reaching the right office or resolving a barrier that prevented follow-through. For the employer, a useful record explains what support occurred and what remains unresolved. A list of telephone numbers supplied is different from evidence that a person reached a service.
Define the position before choosing a course or template. A hospital navigation role, community outreach program and ACHN-related contract may assign different duties. Keep health education within the worker’s preparation and use the employer’s escalation process when a concern needs a licensed professional. A CHW job title alone does not authorize another profession’s services.
Does Alabama require CHW certification?
HB588 was introduced on March 10, 2026 and referred to the House Health Committee. Its introduced text proposed a certification board, competency framework, training approval and a process to seek federal Medicaid approvals. Those provisions describe the proposal, rather than an issued credential or an approved benefit. The Legislature’s official record lists the bill as pending committee action in its House of origin, with March 10 referral entries; the 2026 regular session adjourned April 9. Official bill history, search HB588 in the 2026 regular session. The bill’s proposed October 1 date should not be used as proof of an operating certification program. [3]
This matters when evaluating training advertisements and hiring checklists. Ask who issues an award, what it demonstrates and which employer or contract recognizes it. A course-completion certificate, apprenticeship credential and government-issued professional credential should retain their actual names.
A proposal does not resolve every employer condition. Use the specific job description and funding agreement to identify required preparation, experience, screening and supervision. Avoid telling a worker that an employer’s requirement does not matter merely because it is not a statewide certification mandate.
How can someone become a CHW in Alabama?
Start with a role and training pathway that fit the community and employer. ConnectionHealth is a concrete Alabama training resource, while its ACHIEVE initiative describes workforce development and apprenticeship participation. [2, 4]
- Review the employer’s duties and entry criteria. Ask which learning award and prior experience it accepts.
- Compare the actual curriculum, delivery format, assessment, field experience and support available from the training provider.
- Confirm the current cohort, application process, cost and any funded place directly. A historical grant description is not a promise that a seat or stipend is available today.
- Keep the completion award and supporting evidence, then finish employer orientation and any required supervised practice.
- Obtain the program’s service form, referral process and escalation instructions before working independently.
For experienced outreach staff, ask whether the provider has an upskilling or apprenticeship route. Do not repeat a full course solely because a software profile has only one generic “certified” field. The workforce record should reflect the evidence the actual program recognizes.
What Alabama CHW training is available?
ConnectionHealth’s ACHIEVE initiative describes a partnership with Alabama public-health, education and health-system organizations. Its goals include preparing new CHWs, supporting experienced workers and connecting preparation with employment. Ask for the current curriculum, class schedule, assessment and work-experience arrangements for the particular opportunity. A provider’s course outline should not be presented as a statewide licensing rule. [2]
ACHIEVE stands for the Alabama Community Health Worker Institute for Education, Vocational Training, and Engagement. Its apprenticeship page describes mentor support and no-cost training for that opportunity. Posted entry criteria include a minimum age of 16, a high school diploma or equivalent and the specified citizenship or immigration condition. Confirm current eligibility and availability with the provider before applying. [4]
Ask practical questions beyond the hour total: Who observes skills? Is field experience arranged? What does the certificate say? Can a current employer participate? Which costs, equipment or travel remain the learner’s responsibility? Keep the answers with the enrollment decision.
How does ACHN affect Alabama CHW programs?
Alabama’s 2025 to 2029 Child and Family Services Plan describes ACHN as a care-coordination system across seven regions, linking people, providers and community resources. Its examples include appointment help, transportation connections and referral coordination. These examples describe care coordination, rather than an authorization for every CHW to independently deliver the underlying medical service. A CHW supporting an ACHN-related program needs the actual referral and reporting instructions for that relationship. [1]
Before launching, identify the regional partner, accepted referral channel, required identifiers and response process. Determine who owns a referral after it is sent and how the CHW learns whether the participant reached the intended service. A referral transferred to another organization still needs a clear follow-up rule.
ACHN’s program FAQs include separate reimbursement and social-determinants categories for the October 2024 through September 2029 program period. Use those materials with the actual contract to resolve the organization’s responsibilities. A general national CHW billing chart does not describe the terms of a particular ACHN arrangement. [5]
How should an ACHN handoff be organized?
The state plan says care-coordination referrals can come from providers, recipients or community sources. It describes settings chosen by the recipient, including a provider office, hospital, ACHN office, public location or home. For a CHW program, the practical task is to agree on a workable handoff with the receiving team. Alabama DHR, Appendix D, page 3.
Use the following as a planning aid, rather than an ACHN-mandated form:
| Handoff question | Evidence to agree on with the partner |
|---|---|
| Which team is responsible? | Recipient’s region and the actual receiving organization |
| What help does the person want? | A clear request in the person’s own terms, with relevant contact preferences |
| Has the request arrived? | Receipt or accepted referral status, not merely a sent message |
| What happened next? | Appointment, resource connection, declined support or another recorded result |
| Who follows up? | Named worker, next action and the point at which the partner takes responsibility |
Fictional example: A CHW helps a parent ask for appointment support. The regional team acknowledges the request but has not yet arranged the appointment. Keep the request accepted and appointment pending. Closing the CHW’s sending task should not erase the remaining need.
Can Alabama CHW services be billed to Medicaid?
Payment must follow the actual covered service, eligible provider arrangement or contract. Neither a CHW title nor a training award supplies a standalone claim pathway. The introduced HB588 text proposed a process for seeking federal approval. That proposal is not itself a covered-service instruction or approved claim pathway. [3]
For ACHN-related work, begin with the agreement and current program instructions. Ask which organization receives payment, what service or deliverable is funded, what staff qualifications apply and what record supports payment. Keep those answers separate from the everyday activity log.
For grants, employer-funded outreach or negotiated service contracts, the required output may be an activity report, referral outcome file or invoice. Do not buy claims functionality on the assumption that each outreach contact must become a claim. Do not import a code, unit definition or rate from another state.
A program can document valuable support that does not produce a separate claim. Keep unsuccessful outreach and unresolved referrals visible so the service history remains useful to the team and participant.

What should a program confirm before budgeting for reimbursement?
Build a written funding decision for the actual service arrangement. This is an operating recommendation, not a new Alabama payment rule. It should identify the organization signing the agreement, the covered population or deliverable, the qualified staff, the source of payment instructions and the person who authorizes an invoice or claim.
| Funding arrangement | Question that must be answered |
|---|---|
| ACHN relationship | What does the regional agreement purchase, and which partner approves the work? |
| A specific Medicaid-covered service | Which current service manual and provider category authorize this organization’s claim? |
| Training or workforce grant | Which expenses and reporting periods are funded, and who holds the award? |
| Employer-paid navigation | Which participant outcomes and staff responsibilities define the position? |
A course scholarship pays for preparation under its own terms. It does not establish the price of a participant contact. A regional contract also should not be converted into a statewide per-contact fee schedule. Retain the source and version used for each funding decision so later staff can understand why a payment route was selected.
What supervision and onboarding should a program establish?
Give each responsibility a named owner. Operational supervision should address caseload assignment, documentation review, referral barriers, staff preparation and coverage when someone is absent. If a contract requires another professional relationship, document it separately from ordinary line management.
Use an onboarding checklist matched to the job:
- Accepted training and experience evidence, with issuer and verification date.
- Employer orientation, participant communication practices and record-access permissions.
- Referral destinations, escalation contacts and response expectations.
- Note review, corrections and report approval responsibilities.
- Fieldwork arrangements, device expectations and a process for interrupted work.
These are recommended program controls, not an Alabama-mandated statewide software template. Review them with the organization responsible for the service and funding agreement.
What should an Alabama CHW note include?
Write a record that connects the participant’s goal with what the CHW did and what happens next. Recommended fields include service date, worker, contact method, need addressed, support provided, participant response, referral destination, result and follow-up owner. Record duration when the relevant agreement or service requires it.
Fictional example: A participant wants help getting to a primary care appointment. The CHW reviews transportation options with them, helps contact a resource and schedules a follow-up. The resource has not confirmed the ride. The note should retain the pending status and next contact date rather than say transportation was completed.
A supervisor should be able to return a note for correction and see the retained history. A new worker taking over should find the open task without reconstructing the case from several spreadsheets.
How should referrals and program outcomes be reported?
Define each status before collecting data. Offered, accepted, sent, scheduled and completed are different events. If a participant declines or a resource has no capacity, retain that result and the agreed next step. Do not treat every closed staff task as a successful connection.
Report unique participants separately from contacts and referrals. One person may have several outreach attempts and several resource needs. A useful monthly report can show people reached, completed contacts, referral outcomes and outstanding follow-up under the funder’s definitions.
ACHN publishes quality resources, baselines and technical reports. An organization working within that structure should map its assigned reporting specification to the underlying records. CHW activity counts should not be presented as clinical-quality results without the required measure evidence. [7]
How can software help an Alabama CHW team?
Software is useful when it connects the participant record, assigned worker, referral response and required report. Explore community health workflows to see the program context, then test your own ACHN handoff or grant deliverable.
Use a fictional transportation referral in a walkthrough. Include a failed contact, a resource that cannot help and a reassignment. Ask the team to find the next action and produce the report from the same records. Confirm the actual partner exchange method and any configuration work.

Frequently asked questions
Is ConnectionHealth training the same as a state-issued Alabama license?
Keep the training provider’s award and its purpose clear. Employer recognition of a course is different from a government-issued professional credential. The introduced HB588 text describes a proposed certification framework. [2, 3]
Is every Alabama CHW course the same length?
Compare the actual course and employer requirements. Ask which hours are instruction, assessed practice and supervised work experience. A course total should not be labeled the Alabama licensing requirement solely because a provider uses it. [2]
Does ACHIEVE always provide a free place?
The posted apprenticeship describes no-cost training for that opportunity. Confirm the current cohort, eligibility and offer before budgeting around it. [4]
Does an ACHN referral prove that a medical service was delivered?
A referral records a request or handoff. Retain separate confirmation of attendance or service receipt. The state’s care-coordination examples include helping people obtain services, rather than treating every referral as a completed medical visit. [1]
What should a small program organize first?
Start with a reliable participant list, named follow-up owners, a consistent service note and the funder’s reporting definitions. A software purchase should demonstrate those functions with your actual workflow.
What should an Alabama program do next?
Confirm the workforce preparation, referral partner and funding agreement, then map one participant journey from intake through the report. Preserve open needs and keep payment review distinct from support delivered.
Bring that workflow to Smores Health. Book a program walkthrough to evaluate the participant record, staff responsibilities and reporting requirements together.
