What does a community health worker do in Colorado?

Colorado uses CHW as an umbrella term that includes health navigators, patient navigators, promotores and community outreach workers. CDPHE recognizes a distinct Tribal CHR pathway. The approved future Medicaid framework describes preventive services spanning education, coaching and navigation. CMS-approved future service framework. [2]

For a program director, the starting point is the position’s actual work. Define the population served, languages and community relationships needed, the participant’s goals and the boundaries of the role. A job title alone does not tell a supervisor whether a worker is providing education, coordinating access, carrying out a clinical task or collecting data for a grant.

Write these distinctions into onboarding. Give workers a clear route to ask a clinician for help, escalate an urgent concern and hand off an unresolved need. A registry credential should sit alongside practical employer orientation.

How do you become a credentialed Colorado CHW?

The training route requires a CDPHE-recognized program and the state competency assessment. The experience route recognizes 2,000 hours within five years, permits up to 1,000 volunteer hours and also requires the assessment. CHRs submit relevant training for review. CDPHE currently labels the reciprocity pathway inactive; it plans to assess other states and update the pathway as states are added. [2]

Choose the route before purchasing a course. An experienced worker may need an evidence review, while a new worker needs a complete training plan. Ask the credentialing team which application applies to your circumstances and retain its answer with the application record.

How do the current credential pathways compare?

PathwayPreparation to documentWhat follows CDPHE review
Recognized trainingProgram name, completion date and certificateState assessment, followed by registry listing after passing
Work experience2,000 qualifying hours in five years, competency and employer evidenceState assessment, followed by registry listing after passing
Tribal CHRRelevant current CHR preparation or job descriptionRegistry listing after CDPHE approval
ReciprocityCurrently inactiveDo not assume another state’s certificate bypasses assessment

CDPHE’s current credentialing instructions distinguish the CHR route from the training and work-experience routes. A Tribal CHR should not automatically be sent through a generic new-worker course without reviewing the applicable pathway.

Fictional example: An outreach worker has recent paid and volunteer CHW experience. They should identify qualifying duties and verifiable hours, then submit the work-experience evidence for review. A total above 2,000 does not itself award the credential or eliminate the assessment.

What evidence should an experienced worker prepare?

Build an experience file that identifies employers, dates, hours, duties and the person who can verify them. Keep paid and volunteer hours distinguishable. CDPHE’s current application instructions call for competency evidence, a job description, supervisor attestation and an ethics attestation. [2]

Use a simple reconciliation process: compare the claimed dates and hours to employer records, resolve overlapping positions and obtain the required verification. Do not count every hour in a healthcare job as CHW experience without reviewing the actual duties. This preparation makes the application more reliable and gives the employer a useful record of demonstrated skills.

Where can you find Colorado CHW training?

CDPHE’s workforce-development page is the state entry point. The Patient Navigation and Community Health Worker Training Program describes its recognized course pathway, Certemy verification and subsequent assessment registration. MSU Denver offers a five-course online certificate aligned to Colorado competencies. [3, 4, 5]

Compare the complete pathway, not only the first course. Ask each provider about current recognition, course sequence, field requirements, assessment preparation, language access, total cost and the next available cohort. A course certificate and state credential are separate milestones.

For employers, decide whether coursework occurs during paid time and who covers travel, assessment and application expenses. Put an expected completion date and a named support person in the worker’s development plan.

What should someone compare before choosing a course?

The university and continuing-education routes can have different completion steps. MSU Denver describes five courses and credit-by-exam components, with a C or better and a minimum 2.0 GPA for its certificate. Its page says students must complete the credit conversion before sitting for the state assessment through that route. Those are conditions of the MSU Denver program, rather than a requirement that all Colorado CHWs earn college credit. MSU Denver course and certificate details.

The Patient Navigation and CHW Training Program describes its own sequence of navigation, care-coordination, CHW, interviewing and online modules. It directs applicants to training verification in Certemy before assessment registration. Obtain the full pathway and current course schedule, rather than buying a single class and assuming it completes the credential. Training program pathway.

Ask which expenses are tuition, assessment and travel; which are paid by an employer or scholarship; and what evidence will be issued. Keep a specific financial offer separate from a general program description.

How do the assessment, registry and renewal fit together?

The training provider’s credential page directs applicants to create a Certemy account for verification before assessment registration. CDPHE’s current workforce page describes a five-year credential and revalidation through a profile update before expiration. [3, 2]

Track application submitted, evidence accepted, assessment passed and credential issued as separate events. Use the actual issued credential to record an expiration date. Avoid relying on an anticipated completion date as proof of current status.

For renewals, assign reminders early enough to resolve a changed email address, missing evidence or access problem. Verify current CDPHE instructions before renewal rather than assuming that a previously used process remains unchanged.

How should existing registry members use Certemy?

CDPHE’s migration instructions distinguish updating an existing profile from revalidating a credential at expiration. Existing members use the email already on file and the password-reset process to access their account. The credential certificate is available through the digital wallet. Do not create duplicate workforce histories just because an employer or email address changed. CDPHE workforce and Certemy instructions.

An employer’s tracking system should record the credential actually issued, its dates and the outstanding action. It should not imply an automatic connection to Certemy or display a future assessment appointment as an active award.

When will Colorado Medicaid reimburse CHW services?

CMS approved Colorado SPA 26-0003 on July 24, 2026, changing the CHW benefit’s effective date from January 1, 2026 to January 1, 2028. The approval letter also states that the earlier authority was never operationally implemented. Program leaders should treat the future date as a planning milestone subject to current funding and implementation instructions. CMS approval letter. [1]

An older slide, rule or timeline with a 2026 launch does not establish that payment is available today. Use the approved effective date when interpreting earlier implementation materials; an old projected launch graphic is not a billing instruction. [2]

Before activating a reimbursement workflow, confirm the effective benefit, eligible billing organization, worker enrollment, required recommendation, service definitions and current claims instructions. A planned code or registry entry does not supply all those conditions.

How can a CHW program plan funding during the delay?

Build the operating budget around executed grants, employment support and contracts the organization actually holds. Identify which activities each arrangement pays for, the reporting period and any restrictions on overlapping funding. Keep prospective Medicaid revenue outside the committed operating baseline until the organization has verified a usable pathway.

Useful planning questions include:

  • Which participant services remain funded throughout the current contract?
  • What staffing and supervision costs are covered?
  • What happens to a participant’s follow-up when a grant ends?
  • Which reporting obligations continue after the final service date?
  • Who will approve future payer configuration and staff training?

A funding transition should preserve the participant’s history. Changing a program’s funding source should not rewrite the original purpose or classification of earlier contacts.

What should Colorado CHW supervision cover?

Create a supervision plan around the worker’s duties and the program’s service model. Recommended topics include outreach boundaries, consent and information sharing, documentation quality, referral barriers, workload and escalation. These are operating recommendations, not a substitute for a future Medicaid supervision policy.

For Tribal programs, agree on the organization’s own workforce verification, data access and reporting expectations. Do not assume that a state registry entry resolves the Tribal organization’s governance decisions.

A useful supervisor review asks what remains unresolved for the participant and who owns the next action. It should also identify structural barriers, such as a resource with no capacity, so repeated unsuccessful calls are not mistaken for progress.

What belongs in a Colorado CHW encounter note?

Use a concise record that explains the participant’s stated need, what the worker did and what happens next. Suggested fields are the service date, worker, program, contact method, participant goal, activity, response, referral status and follow-up owner. Add time, recommendation and review fields when the governing contract or verified payer rule requires them.

For example, “housing referral completed” may hide several different events. Record whether the worker provided information, sent an application, received agency acknowledgment or confirmed that the participant obtained assistance. If the resource declined the referral, preserve the reason and agreed alternative.

Avoid filling the note with unrelated sensitive detail. The purpose is to support continuity and explain the work, while respecting the organization’s access and disclosure policies.

Which outcomes should a Colorado CHW program measure?

Separate unique participants, contacts, referrals and confirmed outcomes. Three calls to one person are three contacts, not three people served. A submitted referral is a process measure; successful access is a different result.

Define the reporting denominator before comparing programs. For referral completion, specify whether the denominator includes all referrals, accepted referrals or only those old enough for follow-up. Record unknown outcomes honestly instead of converting them to success.

Add a short review of unresolved needs and partner capacity. This gives a director information for service planning and partnership discussions, even when an activity has no immediate reimbursement path.

What should Colorado CHW software help a program lead manage?

Look for a connected view of participant goals, worker assignments, encounter records, open referrals and reporting. Workforce evidence should identify the actual credential pathway and status. Funding fields should distinguish current contracts from future plans.

Test whether a supervisor can find an overdue task, reassign it and retain the handoff history. Ask the vendor to reproduce a submitted report after a correction without silently changing the original reporting record.

How should you evaluate a Colorado CHW software demo?

Use a fictional worker applying through the experience route and a participant waiting for a resource. Ask the vendor to:

  1. Store the worker’s evidence without marking an unissued credential active.
  2. Document the participant’s goal and current grant-funded contact.
  3. Record a pending referral, assign follow-up and show the supervisor’s queue.
  4. Correct a note and trace the revised report to its source record.
  5. Describe how a future funding change would be approved without altering earlier services.

Frequently asked questions

Is Colorado’s CHW registry the same as Medicaid provider enrollment?

No. Credentialing establishes workforce status. A reimbursement program also needs operational coverage and enrollment instructions. Keep the two decisions separate when preparing for the postponed benefit. [1, 2]

Can volunteer experience count toward a Colorado CHW credential?

The work-experience pathway permits up to 1,000 of the required 2,000 hours to be volunteer work within the relevant five-year period. Keep verification for both paid and volunteer roles. [2]

Does an out-of-state certificate automatically transfer?

No. CDPHE currently labels reciprocity inactive. Do not promise a transfer or assessment waiver based only on holding another state’s certificate. [2]

Can a training certificate replace the competency assessment?

The Colorado training pathway includes both recognized training and a passing assessment. Track each milestone rather than treating course completion as the issued state credential. [2, 3]

Should software be purchased using projected 2028 claims revenue?

Base the purchase on current operational value and confirmed funding. Make any future reimbursement scope contingent on verified implementation, organization eligibility and payer instructions. [1]
