Hawaii CHWs can earn college certificates while statewide professional standards continue to develop. A college Certificate of Competence is distinct from state certification and Medicaid payment eligibility. This guide compares training resources, explains the standards task force’s work and helps program leaders manage culturally responsive services, referrals, documentation and software. [1, 2]

What does a community health worker do in Hawaii?
Hawaii’s DOH report describes CHWs as intermediaries between communities and health or social services, with work spanning outreach, education, advocacy and support. It recognizes the breadth of roles across community, cultural, faith-based, public-health and healthcare organizations. [5]
For a program lead, that breadth calls for a clear local service model. Define the community served, language and cultural knowledge needed, participant goals and the team’s clinical escalation route.
A trusted worker may know why an otherwise reasonable referral will not work for a family. Make room for that knowledge in care coordination and program decisions rather than treating it as information outside the formal workflow.
Does Hawaii require state CHW certification?
Kapiʻolani Community College’s current FAQ states that Hawaii does not issue a state CHW certification and distinguishes its own Certificate of Competence. DOH’s task-force materials document work on professional standards. [1, 2]
Use the award’s actual issuer and title in workforce records. College completion, employer approval, a voluntary association credential and a government license are different categories.
Particular employers, training programs and service agreements may set qualifications. Verify those conditions for the role instead of applying a mainland state’s certification or renewal requirements to every Hawaii CHW.
What did the CHW professional-standards task force establish?
The task force formed in June 2025 under SCR16 SD1 and sunset in December 2025. DOH published a report to the 2026 Legislature. Its foreword explains that stakeholder views differed and that the report presents those views rather than a single completed set of uniform recommendations. [1, 5]
The report and its surveys should not be treated as a live application manual. An individual member’s preferred credential name, training threshold or fee does not become a statewide rule because it appears in an appendix.
Monitor implementation separately. Before describing a new credential as available, identify the actual issuer, operative requirements and application route.
Where can you get CHW training in Hawaii?
University of Hawaii programs provide several starting points. Kapiʻolani lists a 16-credit Certificate of Competence. Maui’s catalog lists a 15-credit CHW/Health Navigator I certificate. Hawaii Community College offers its own CHW certificate and entry conditions. [2, 3, 4]
Compare the program that fits the worker’s location and schedule. Ask about delivery format, prerequisite courses, placement, fieldwork, tuition, financial aid and the next cohort. Credits and course sequences vary by institution.
For an employer, training selection should connect to duties. A worker providing outreach and navigation may need additional orientation for a specialized maternal, chronic-condition or palliative-care program.
What is the current Kapiʻolani scholarship opportunity?
As reviewed November 6, 2026, Kapiʻolani advertises a Spring 2027 DOH-sponsored cohort with scholarships for up to 20 students, covering tuition and student fees. Its deadline is December 4, 2026, and the opportunity includes a 120-hour DOH practicum in Fall 2027 with weekday availability conditions. Admission and scholarship selection are not guaranteed. [2]
Read the live program page before applying because cohort details can change. Ask how the practicum schedule fits with employment and what other expenses remain.
For program leaders sponsoring staff, avoid promising a scholarship or placement until the responsible organization confirms it. Keep this dated opportunity separate from the ordinary cost of CHW training statewide.
How do Maui and other college programs differ?
Maui’s CHW/Health Navigator I catalog includes fundamentals, counseling, case management, fieldwork and health promotion within its 15-credit certificate. [3] Hawaii Community College’s program page identifies required reading and writing placement levels for relevant courses. [4]
Ask an advisor which catalog and entry rules apply to the student’s cohort. A historical flyer can describe an older course sequence or grant-supported offering.
When comparing programs, distinguish the credit total from contact or internship hours. Do not combine a fieldwork requirement from one college with another college’s credit total and call the result a statewide standard.
What should a staff member compare before choosing a college?
| Program | Posted award | Planning decision |
|---|---|---|
| Kapiʻolani | 16-credit Certificate of Competence | Review the cohort schedule, required practicum and any separate scholarship selection |
| Maui | 15-credit CHW/Health Navigator I Certificate of Competence | Confirm fieldwork, course availability and the applicable catalog with an advisor |
| Hawaiʻi Community College | CHW Certificate of Competence | Check reading and writing placement for the intended courses |
Maui’s catalog PDF lists fundamentals, individual counseling, case management, fieldwork and health promotion/disease prevention. Hawaiʻi Community College identifies reading placement into English 102 and writing placement into English 100 for relevant program courses. Those academic conditions belong to the colleges, not to a universal statewide certification application. Maui catalog PDF and Hawaiʻi Community College.
For an employee comparing routes, put tuition, travel, available work time, practicum hours and the exact award on one page. Ask the advisor to confirm the individual plan before the employer promises that a course will fit a particular grant or payer requirement.
How should a Hawaii employer manage workforce development?
Maintain a record of the worker’s role, training issuer, courses, field experience, language skills, orientation and supervision assignment. Record the actual award date and any employer-required refresher dates.
Use a development plan with practical demonstrations: a respectful participant intake, an accurate referral handoff and a concise note. Review how the worker handles a question outside their role.
For Native Hawaiian and Pacific Islander-serving organizations, agree on culturally appropriate engagement and data practices with the organization and community. A generic form should not erase the context that makes the service useful.
How do CHWs fit into Hawaii Medicaid?
CMS lists Hawaii QUEST Integration as an approved Section 1115 demonstration. That status does not mean every CHW contact has an individual reimbursement code. [6]
The CMS-approved Hawaii palliative-care amendment also recognizes CHWs as optional members of a palliative-care interdisciplinary team. This is a specific team context, rather than a universal independent CHW billing category. [7]
For a particular program, establish the actual Med-QUEST or health-plan arrangement: covered service, eligible organization, staff conditions, payment basis and documentation. Separate team funding, employment, grants and any verified claimable services in the operating budget.

How is a palliative-care team role different from independent CHW billing?
The CMS-approved palliative-care amendment lists the CHW as an optional interdisciplinary-team member for adult and pediatric care. Its role includes outreach, community education, support and advocacy, with lived experience and trusted community membership identified in the team table. The table allows the state to require additional qualifications; it does not itself issue a separate statewide CHW credential. Approved SPA 22-0013, Supplement 4, page 2.
The same amendment uses a monthly service bundle paid to the qualifying provider. It permits only one bundled payment provider per beneficiary, requires at least one included service during the payment unit and prohibits separately billing services already included in the bundle. Those are conditions of that benefit, not permission to submit an independent claim for every outreach contact. Approved SPA 22-0013, Attachment 4.19-B, page 1.2.
For a CHW employed on such a team, clarify who owns the care plan, where the CHW documents their contribution, how concerns reach the clinical team and which organization maintains billing evidence. This preserves the worker’s useful navigation role while keeping clinical services and payment accountability with the responsible professionals and provider.
What funding questions should a Hawaii CHW director answer?
Document the agreement supporting each program and the costs it covers. Consider staff time, travel, inter-island coordination where relevant, supervision, language access, training and reporting.
Ask whether the arrangement pays for capacity, participant engagement, completed services or outcomes. Identify reporting deadlines and what evidence supports each measure.
Keep proposed funding outside committed revenue until approved. A new standards initiative, scholarship or demonstration document may support planning while still requiring separate contracting and implementation.
How should island and language access shape referrals?
Track the resource’s actual service area and intake method. Confirm whether a participant can use the service from their island, whether remote access is offered and whether language support is available.
Record the participant’s preference and barriers rather than assuming a directory listing is accessible. If travel or scheduling prevents access, keep the need open and identify the next action.
A useful partner record includes contact, location, language access, referral requirements, capacity information and the last verification date. Give workers a simple way to flag a listing that no longer works.
What should a Hawaii CHW encounter note contain?
Suggested fields include the participant’s goal, service date, worker, program, contact method, language support, activity, participant response and next step. Add the particular service’s time, consent or review requirements.
A fictional referral note might state that the participant accepted a specialty-care referral, the destination requires an additional intake step and the CHW will help obtain the requested information. Do not mark assistance received while that step remains unresolved.
Use the minimum detail needed to support continuity and explain the work. A supervisor should understand the barrier and task owner without reading unrelated sensitive history.
What should supervision and reporting focus on?
Review unresolved needs, participant preferences, referral quality, staff workload and escalation. Keep training progress and employer-required education visible in a separate workforce view.
For reports, distinguish unique people from repeated contacts and referrals sent from confirmed access. Define the period and denominator for completion measures. Keep unknown outcomes explicit.
Review partner capacity and recurring access barriers alongside service totals. This gives a director evidence for improving relationships and resources, rather than treating every incomplete referral as an individual worker’s failure.
How do you choose Hawaii CHW software?
Test the connected workflow from participant goal to documentation, referral follow-up, supervisor review and reporting. Include the actual devices and working conditions your staff use.
Explore community health workflows and the Smores Health platform. Bring a blank form, an island-specific handoff and a required report to evaluate fit. Ask the vendor to demonstrate current functionality and name any configuration needs.
Do not infer an integration or offline capability from a general product description. Confirm the data flow, permissions, failure handling, implementation scope and export process.
What should a Hawaii program test in a demo?
Use a fictional participant who needs a resource on another island and prefers a particular communication language.
- Record the goal, service program and communication preference.
- Identify the resource’s service area and remaining intake conditions.
- Document the contact and leave the referral pending until evidence supports its outcome.
- Assign a backup worker without losing the plan.
- Produce a report separating contacts, referrals and confirmed access.
