Missouri Community Health Worker Guide

Learn Missouri CHW-C certification, approved training, fees, renewal and Primary Care Health Home funding, with documentation and software guidance for leaders.

Published by Smores HealthReviewed

Missouri separates approval of CHW training providers from individual CHW-C credentialing through the Missouri Credentialing Board. Program leaders must also distinguish workforce credentials from MO HealthNet program participation. This guide covers training, application evidence, renewal and the Primary Care Health Home context, with practical documentation and software evaluation guidance. [1,2,6]

What does a community health worker do in Missouri?

DHSS describes CHWs as community-connected workers who help people access health and social services, build health knowledge and support community capacity. Outreach, education, social support and advocacy are part of the broader role. [8]

Employers should define the population, service area, referral relationships and supervisor before hiring. Make clinical escalation and confidentiality expectations clear. A credential helps establish workforce preparation, while the job description explains what the worker is expected to do in the actual program.

Who certifies Missouri CHW training and individual workers?

DHSS certifies curriculum providers. A worker completing a program through a certified provider receives a completion certificate and then applies separately to the Missouri Credentialing Board for individual credentialing. DHSS explicitly describes those separate steps. [1]

Do not report every course graduate as already holding CHW-C. Workforce records should distinguish enrollment, course completion, MCB application, issued credential and renewal status.

What training is required for CHW-C?

The current MCB application requires completion of a DHSS-approved CHW standard training program. DHSS’s current provider application describes 100 classroom hours and 60 service-learning hours, reduced to 40 service-learning hours for a trainee currently employed as a CHW. Check the approved provider and the actual course before enrolling. [3,4]

A specialty workshop can build skills without replacing foundational training. Ask how a program documents attendance, learning assessment and field experience, and whether its completion evidence is accepted for the CHW-C application.

How should learners compare Missouri programs?

Use the DHSS curriculum overview and provider resources to identify current training options. Confirm cohort dates, cost, delivery format and placement support directly. A provider's approval status does not guarantee an available seat on a particular date. [1,4]

For working learners, clarify how fieldwork fits their schedule and whether the employer can host it. Ask what happens after a missed session or incomplete placement. Preserve the exact program name and completion date in the worker's file.

How should service learning and delivery format be checked?

DHSS’s current provider application says classroom instruction may combine in-person and online learning. It recommends a majority of synchronous instruction but does not make that recommendation an absolute requirement. Academic credit is also optional. Learners should confirm actual live-session requirements and whether credit is awarded before choosing a course. DHSS provider application, pages 1 to 2.

The same document requires approved curriculum and describes competency evaluation and service-learning support. Its 40-hour provision for an already-employed CHW concerns service learning, rather than reducing the 100 classroom hours. Ask the program to confirm the placement, applicable hours and completion evidence in writing. DHSS provider application, pages 3 to 5.

What does the MCB application require?

The application packet identifies high school diploma or equivalency, approved training, a completed application, ethics agreement, fee and Family Care Safety Registry materials. It also provides instructions for education evidence, background review and exceptions. [3]

Use the current packet, answer disclosures accurately and retain copies. If an exception or review is needed, follow MCB's process rather than relying on an employer's informal assurance that the issue will not matter.

What should applicants know about paper submission and background evidence?

The application page instructs paper applicants to mail the packet rather than fax or email it. The current linked packet, revised December 2025, requests the signed ethics agreement, education and training evidence, and Family Care Safety Registry material. An agency’s FCSR results from the preceding 30 days may help expedite review, but the packet’s instructions still apply. Current MCB application page and packet, page 1.

The packet gives a typical two-week review period and asks applicants who have received no correspondence three weeks after mailing to contact MCB. That is a planning estimate, rather than a guaranteed credential date. Preserve the complete submission and respond to the issuer’s actual request if review is needed.

How much does Missouri CHW-C certification cost?

MCB's current application page lists an $82.50 registration fee. Training tuition and other expenses should be budgeted separately. The page offers an online application and explains how to submit the paper form. [2]

An employer supporting certification should specify whether it covers tuition, learning time, application costs and travel. Review the actual provider quote and current Board fee before promising a fixed total to a new hire.

How can employers verify an issued credential?

The MCB application directs applicants to its Professional Search to check credential information when an application is complete. Retain the credential identifier, issue or expiration information and verification date. [3]

A pending application should remain pending in the staff record. If a worker renews or returns from a lapse, verify the updated status rather than carrying forward the previous expiration date.

How does Missouri CHW-C renewal work?

MCB's renewal requirements list 20 total hours, including six ethics hours, for CHW-C. Its general guidance describes an every-other-year cycle after the first renewal and explains that first-renewal learning requirements depend on the initial credential letter. [5]

Track course date, hours, subject and completion evidence during the cycle. Ethics learning should be identified explicitly so a supervisor can see the remaining requirement. Do not assume that another MCB credential's education total applies to CHW-C.

Must ethics continuing education be completed live?

MCB’s published renewal requirements say ethics hours no longer have to be live. They permit live, in-service, online and self-study formats when completion evidence identifies the hours and date. All renewal learning must fall within the worker’s current cycle. MCB renewal requirements.

For a worker’s first renewal, use the original award letter to establish the individualized requirement. For later cycles, track the 20 total hours and six ethics hours with their completion evidence, without counting an enrollment confirmation as attendance.

What if a Fall 2026 renewal was missed?

The current Fall 2026 page says submissions after October 31, 2026 require reinstatement and applicable fees. It also explains earlier processing-fee windows. As of this guide's review date, that ordinary fall renewal deadline has passed. [7]

Use the issued credential's actual expiration and contact MCB about reinstatement. A saved renewal draft or an employer's receipt of education records does not prove that the credential has been renewed.

How does MO HealthNet's Primary Care Health Home model fit?

MO HealthNet describes PCHH as a model for comprehensive care management and coordination for a medically complex population, including attention to social determinants of health. It publishes program enrollment, staffing and implementation resources. [6]

The current initiative page links approved SPA 25-0021. The final approved package records approval on April 27, 2026, with the relevant service units effective October 1, 2025. Use that approved evidence and current operational guidance rather than an older draft amendment. [9]

What role does the CHW hold within the PCHH team?

The approved plan places CHWs within a physician-led practice team and includes their time in the per-member-per-month payment. The CHW section identifies outreach, regular care-team communication, education, referral tracking, assistance with access barriers and support for participant goals. These are responsibilities within the health-home model, rather than a separate payment for every CHW contact. Approved SPA 25-0021, Health Home Providers, Provider Infrastructure.

For a fictional participating practice, an agreement with a community partner can identify who follows up on a housing-related access barrier, what progress returns to the practice and how the team avoids losing the handoff. Record the actual navigation and outcome separately from the practice’s health-home payment eligibility. Confirm the current participation and reporting terms before projecting revenue.

Does CHW-C authorize independent fee-for-service billing?

The credential alone does not establish a separately payable encounter or authorize any organization to submit claims. A health home team payment, managed care contract and standalone service benefit are different arrangements.

Before setting revenue targets, identify the participating organization, attributed population, staffing conditions, covered activities and required reports. Use a written agreement for a community partner's role. Preserve time and services accurately without counting the same work under several funding sources.

What should a Missouri CHW encounter record show?

As a program-design starting point, document the participant's goal, need addressed, support provided, response and next action. Include date, worker, contact method and referral owner, adding payer-specific requirements where applicable.

A referral should retain its actual status. A resource can be suggested, accepted, sent, waitlisted or completed. A closed staff task does not necessarily mean the participant received the service.

How should supervisors connect workforce and service quality?

Review credential readiness alongside caseload and support needs. A staff member approaching renewal may need learning time; a worker managing repeated failed referrals may need help from a partner or clinical team.

Use supervision to improve documentation and problem-solving. Keep clear assignments and correction history so another worker can continue follow-up when the original CHW is unavailable.

What should Missouri CHW software demonstrate?

Test a worker moving from training completion to MCB approval, then a participant supported through a practice or community partnership. Confirm that qualifications, assignments, notes and outcomes remain distinct and visible.

Which reports should program directors trust?

Separate course graduates, credentialed workers, people served, contacts and confirmed outcomes. For a health home partnership, use the participation and reporting definitions required by the actual program.

Trace a reported total to its source records. Test a corrected note, an expired credential and a participant with multiple contacts. Compare the software's total cost with the implementation work needed to produce dependable reports.
