Rhode Island Medicaid requires fully certified community health workers with individual NPIs, group affiliation and the applicable enrollment checks. Services require individualized clinical authorization and documentation. The current EOHHS page also rescinds the CHW electronic visit verification requirement effective July 1, 2026, while retaining documentation and safety expectations. What does a CHW do in Rhode Island?
CHWs use trusted relationships to connect people with health information, services and community resources. RICB describes education, engagement, social support and advocacy as important parts of the role. A worker’s community knowledge helps make support understandable and responsive to the person receiving it. RICB credential requirements
EOHHS defines CHW services as nonclinical support. Before a covered visit, identify the authorized goal, the clinical recommending practitioner and the worker’s actual intervention. EOHHS provider manual
Rhode Island CCHW certification requirements
RICB’s current CCHW page identifies education, qualifying experience, on-the-job supervision, a signed job or volunteer description and a portfolio. The downloadable application provides the evidence details. A training completion certificate alone does not establish that all credential requirements have been met. RICB credential requirements RICB application
Before applying, organize the experience and supporting materials by requirement. Keep work hours separate from supervision hours. If experience spans several employers, obtain the relevant verification rather than estimating from a résumé.
Preparing the certification portfolio
The RICB application requires three distinct portfolio categories, with its first category mandatory. Repeated submissions within a single category do not satisfy the three-category requirement. Review the current instructions before gathering evidence so the applicant can select materials that actually meet the chosen categories. RICB application
An employer can help a worker preserve appropriate evidence without placing identifiable participant information into an unrestricted portfolio folder. Use the credential administrator’s instructions and the organization’s information-handling process to determine what may be shared.
How to apply and budget for certification
- Review RICB’s current CCHW requirements and application.
- Collect education, experience and supervision evidence.
- Obtain the signed job or volunteer description.
- Complete the required portfolio categories.
- Submit the application, supporting materials and fee.
- Verify the issued credential and expiration.
The application currently linked by RICB lists a $125 certification fee and a two-year recertification cycle. Confirm current renewal requirements and fees directly with RICB before planning the next cycle. Do not borrow education totals from another state’s CCHW credential. RICB application
Medicaid enrollment and group affiliation
EOHHS’s November 2025 FAQ explains that both the group and each CHW need their own NPI. Individual workers must affiliate with a Medicaid-enrolled group, independent enrollment ended under the revised model. Certification, enrollment and affiliation are separate items to verify. EOHHS enrollment FAQ
A staffing tracker should show the worker’s credential and enrollment dates, the group affiliation and any unresolved condition. An NPI is an identifier, not evidence that an enrollment is approved. Check the organization and worker status for the actual service date.

Full certification and screening
The current agency timeline closes transitional certification and requires full RICB certification for continued Medicaid eligibility after the 2025 transition. It also identifies national criminal-background checks as an enrollment condition. Do not describe the old transition period as an available route for a new Medicaid worker. EOHHS changes and timeline
Give onboarding tasks a clear owner and keep sensitive screening information appropriately restricted. Managers generally need the verified status and relevant dates, a broad staff roster should not become a repository for unnecessary screening details.
Individual LPHA orders and medical necessity
The provider manual requires a formal order before services, issued by an eligible Rhode Island-licensed, Medicaid-enrolled LPHA with direct clinical knowledge. Generalized standing orders and retrospective diagnosis are not substitutes. The order must connect the authorized CHW service with the participant’s condition or risk. EOHHS provider manual
At intake, check that the order belongs to the right participant and addresses the proposed work. If the order is unclear, obtain clarification before treating a generic referral as authorization. Preserve the original order and subsequent updates so a reviewer can understand the service’s basis.
Can a community class be billed as a Rhode Island CHW group?
A public workshop can serve the community. To bill covered group coaching or education, the facilitator must verify each Medicaid beneficiary’s individual need and authorization, keep the group at eight or fewer per single CHW facilitator, and document each person’s engagement. A sign-in sheet alone cannot establish the link to a participant’s goal. EOHHS provider manual
For example, two people might attend the same class on following a care plan. One practices using their own appointment instructions. The other asks a question about the next step for their own goal. Record those actions separately and tie each to their own order. If another person attends as an observer, keep their attendance out of the covered count. EOHHS provider manual
Service categories, time and limits
The manual distinguishes individual or group coaching and education from individual navigation. For navigation, H0038 is billed in 15-minute units. S9445 and S9446 are session codes. The manual describes a 30-minute session with at least 25 minutes of actual service and prohibits rounding up to reach the minimum. Apply the correct service-specific modifier and document the actual time. EOHHS provider manual
| Activity | Aggregate limit without prior authorization |
|---|---|
| All CHW services combined for one person | Two hours per day and 12 hours per calendar month. Count time across categories. EOHHS changes and timeline EOHHS provider manual |
| Navigation only, expressed in 15-minute units | Eight units per day and 48 per month. EOHHS provider manual |
| Session-based services only | Up to four sessions per day and 24 per month, within the same combined time limits. EOHHS provider manual |
If the proposed service exceeds the daily or the monthly threshold, follow the prior authorization process before exceeding it. The session and navigation figures describe alternative ways to express the same overall limits. EOHHS provider manual
What changed with electronic visit verification?
EOHHS rescinded the CHW electronic visit verification requirement effective July 1, 2026. Its September 2026 program update supersedes the EVV language in the February provider manual. Continue to document the home visit and follow the home-visit safety standards. A time record still matters for the cumulative service limit. EOHHS changes and timeline EOHHS provider manual

What should contact documentation show?
A useful note connects the authorized purpose with the actual contact, action, participant response and next step. Record the worker and date, and preserve timing and modality information where required. A note should help the next team member understand what remains unresolved.
Suppose a participant needs help navigating a follow-up appointment. The first contact may identify the barrier and agree on an action. A later contact may confirm whether the appointment was scheduled or attended. Keep those events distinct so a referral sent does not become an unsupported successful outcome.
Review a Rhode Island visit before it enters the billing queue
Consider an enrolled group that has already delivered 90 minutes of authorized work for one beneficiary today. The order supports another 45-minute visit. Together the visits would be 135 minutes. The daily standard is 120 minutes even when monthly use remains low. Follow the prior authorization process or arrange a permitted alternative before exceeding the limit. Keep the beneficiary’s preference and the clinical reason for the follow-up in the record. EOHHS changes and timeline EOHHS provider manual
At the end of the month, review group affiliation, the order, the contact date and duration, the service category, the beneficiary’s response and any prior approval together. For home visits, keep the organization’s safety steps visible. To check a community health documentation workflow, use an invented member who had both a group session and a navigation visit in the same week.
