Washington launched its Apple Health community health worker benefit on July 1, 2025. The state does not issue a general CHW certification, but Medicaid has specific worker, supervision, recommendation and documentation requirements. Program leaders should use the current billing guide for service dates and keep course completion separate from payment eligibility. HCA CHW benefit Washington DOH training WAC 182-562-0400

What does a CHW do in Washington?
Washington’s CHW framework emphasizes trusted community relationships and prevention through education, navigation and support. Community Health Representatives are recognized as CHWs for the benefit’s purposes. The actual service and provider arrangement still determine the applicable requirements. HCA CHW benefit WAC 182-562-0400
For program design, define the community served, worker responsibilities and escalation contacts. A training certificate should not be used to represent a worker as a licensed clinician or as holding a separate peer-support credential.
Does Washington require CHW certification?
DOH says Washington does not require CHWs to be certified and has no official CHW certification or credential process. Its courses provide completion certificates. Medicaid qualification is a separate question governed by HCA’s requirements. Washington DOH training
A workforce record should therefore identify the actual course and issuing organization, along with the evidence supporting the worker’s Apple Health role. Avoid a single “state certified” field that misstates the qualification.
Washington CHW training resources
DOH offers free online core, specialty and pediatric courses, with many available in English and Spanish. Its core course uses self-paced modules and a completion quiz. The page advises people pursuing Medicaid eligibility to confirm with their employer or HCA which course they need. Washington DOH training
Before enrolling staff, compare the course with the role, existing skills and applicable payer requirements. Completion is useful evidence, but it should not be treated as an automatic approval for every service or billing arrangement.
Apple Health worker qualifications
WAC182-562-0400 addresses general supervision by an enrolled licensed practitioner, relevant lived experience, 2,000 supervised CHW hours within three years, core competencies and evidence through a completion certificate or supervisor assessment and attestation. It also provides an 18-month period to obtain missing skills or practical training, with conditions on continued payment. WAC 182-562-0400
These provisions should be read together. Do not simplify them into an unconditional choice between a short course and experience, or treat the training period as a waiver of every other requirement. Obtain HCA clarification for a worker whose circumstances do not fit the written instructions.
Documenting qualification and ongoing education
The worker rule requires at least six hours of additional training annually and directs the supervising provider to retain education evidence. Supervisor attestation also requires documentation of the assessment supporting it. WAC 182-562-0400
Build a workforce record with the experience evidence, course completion, assessment, supervisor, relevant dates and annual education. Preserve what was actually verified. An unchecked training gap should remain visible rather than being converted into a guessed approval date.
Recommendations and initiating visits
WAC182-562-0300 requires services to be initiated and recommended by an appropriate licensed practitioner. Its initiating-visit process identifies the need and includes a written recommendation, which may be maintained electronically. The practitioner personally performs the initiating visit. WAC 182-562-0300
Before the CHW begins funded work, check that the recommendation applies to the right participant and proposed need. A general resource referral should not automatically be treated as the complete benefit authorization.
When can an Apple Health member be considered for CHW services?
The rule starts with an eligible Apple Health program and excludes Medical Care Services (MCS). At a personally performed initiating visit, the licensed practitioner identifies at least one qualifying criterion, creates a care plan and provides a written recommendation. The rule permits electronic or physical written recommendations. WAC 182-562-0200 WAC 182-562-0300
Examples of qualifying criteria include an unmet health-related social need limiting access to care, a positive adverse childhood experiences screen, two or more missed health appointments in the previous six months, a need for recommended preventive care, or a request from the member, spouse or family member for health system navigation or resource coordination. Other criteria address a serious high-risk condition with specified risks or a condition needing ongoing care-plan adjustment. A referral can start a conversation. Check the initiating visit, eligibility and planned service before billing. WAC 182-562-0200 WAC 182-562-0300
Supervising-provider responsibilities
The supervising-provider rules address enrollment, understanding the CHW role, facilitating training and maintaining supervision and performance records. Operational management and clinical accountability should be clear in the organization’s staffing model. WAC 182-562-0800
Give workers a way to obtain timely guidance and escalate a changing need. A manager may review documentation and scheduling while the qualified supervising provider carries the applicable responsibility. Avoid one ambiguous “supervisor” label that hides who does what.
Washington CHW documentation requirements
WAC182-562-0500 requires advance consent, contact date and duration, service location, activity, individual or group status, group size where applicable, identified needs and worker attribution. Consent may be verbal or written and must precede services and billing. The rule also points to the billing guide for additional detail. WAC 182-562-0500
A useful record should show the participant’s response and next step as well as required fields. Linking a contact to a need makes it easier for another team member to continue the work without repeating the intake.
Billing changed after the benefit launch
HCA’s December 2025 update materials describe the 2026 monthly base and add-on code structure and use of the clinic’s billing NPI. Base and add-on codes have a defined relationship, an add-on should not be treated as an independent generic visit code. HCA transition briefing
HCA’s current index now lists an October 1, 2026 CHW guide. Use that current guide and applicable fee schedule to confirm code eligibility, time aggregation, modifiers, nonduplication and payer handling for a specific service date. HCA billing guide index

How does Washington’s reduced-time CHI billing option work?
The October 2026 guide distinguishes Community Health Integration (CHI) from Principal Illness Navigation. For CHI, the standard G0019 base is 60 minutes in a calendar month. HCA permits G0019 at 30 minutes with modifier 52. Its table says using modifier 52 prevents use of the G0022 add-on. G0022 is an additional 30 minutes and cannot be submitted alone. Confirm the full code-specific criteria, medical necessity, consent and service-date instructions before submitting. October HCA billing guide
Suppose an organization has documented 30 qualifying minutes for one person in October. The billing team should review the reduced-service option. It should not attach an add-on or count time from a different month to make up a base service. An ordinary referral entry without the qualifying CHI activity, consent and time is insufficient. October HCA billing guide
Check the participant and payment arrangement
A CHW benefit exists, but a particular contact still needs to meet the applicable coverage and provider requirements. Identify the participant’s plan, the supervising and billing arrangement, the service category and any required approval.
For budgeting, include outreach attempts, documentation, travel, supervision and training. A monthly payment structure may support work differently from a per-contact arrangement. Confirm what the agreement actually funds rather than multiplying every interaction by a published rate.
Before sending an Apple Health CHW claim
-
Confirm the member meets a qualifying criterion and has eligible coverage. Keep the personally performed initiating visit, care plan and written recommendation together. WAC 182-562-0200 WAC 182-562-0300
-
Verify the supervising licensed provider, supervised work history, relevant lived experience and the route used to substantiate competencies. WAC 182-562-0400 WAC 182-562-0800
-
Document advance consent and the actual service, time, location and outcome. Match the documented work to the correct code and calendar month. WAC 182-562-0500 October HCA billing guide
-
If the CHI base is reported at 30 minutes with modifier 52, verify that G0022 is absent. Check the service-date guide and the member’s actual payer arrangement. October HCA billing guide
A referral example that tests workflow quality
Suppose a participant needs help attending follow-up care and accessing a community resource. The CHW discusses the barrier, identifies options and agrees on the next action. The record should preserve the participant’s preferences and what was actually delivered.
If the referral is unanswered or the resource is unavailable, retain the history and assign follow-up. “Referral sent” should not imply that the participant received the service. Supervisors need to see the unresolved step and the person responsible for it.
A Washington referral after repeated missed appointments
A clinic has documented two missed visits in five months. The member explains that confusing instructions and unpredictable transportation have made it hard to return. That history fits one criterion the state lists for a CHW referral. The licensed practitioner still personally conducts the initiating visit, establishes the care plan and issues the written recommendation. The team verifies eligible coverage before the service. WAC 182-562-0200 WAC 182-562-0300
The worker or qualified professional obtains advance consent. During follow-up the CHW records the actual activity, time, location and outcome. For instance, the member may practice explaining the transport request and choose to schedule the ride the next morning. The worker records that decision and follows up on whether access occurred. Before billing, compare the service to the precise October guide, since a qualifying referral does not alone establish the criteria for a particular code. WAC 182-562-0500 October HCA billing guide
