New Hampshire’s approved Medicaid amendment covers qualified doula support, with an eight-visit prenatal and postpartum framework and additional visits through prior authorization. OPLC certification is designed for direct Medicaid reimbursement rather than every form of doula practice. This guide explains certification routes, renewal activity, recommendation evidence and enrollment review.

Is New Hampshire certification required for all doula practice?
RSA 310-A:222 establishes a voluntary process for Medicaid reimbursement. Plc 1900 says its scope is direct Medicaid reimbursement. Avoid presenting that credential as a universal prerequisite for every privately funded doula arrangement.
What qualification routes does OPLC recognize?
Plc 1903 allows certification within five years from an organization meeting NDCB standards, or documented training and practice. The latter includes at least ten hours across core competencies, three births in the preceding five years and evidence of at least one perinatal visit annually after training. Use the actual rule and current application for the complete evidence.
The rule addresses good moral character, supporting information and the application process. It does not require a criminal background check under this chapter. Before buying training, compare the proposed credential or transcript with the route you intend to use.
How should a New Hampshire doula prepare the application?
Choose the supported route
Collect the qualifying certificate or the transcript and practice evidence. Preserve dates and issuer information. A course that covers some competencies may still leave evidence gaps.
Submit and retain the decision
Use current OPLC instructions and the fee schedule referenced by the rule. Save the application, returned requests and actual approval. An application confirmation is not the certification itself.
Complete the separate Medicaid arrangement
Confirm provider enrollment, identifiers, effective dates and the applicable payer submission route. CMS’s benefit effective date does not prove an individual provider was approved on that date.
What evidence supports the training-and-experience route?
Plc 1903.04 and 1903.05 ask for the training program’s identity, location and website, plus city and state information for the required births and annual perinatal visits. The rule permits confirmation through relevant employment or Medicaid visit documentation, or the applicant’s attestation. Keep the training transcript and practice evidence aligned to the selected route.
Use a concise evidence index rather than sending unrelated complete client records by default. Identify each requirement, its supporting document and any question for OPLC. The chapter waives its application fee for active military applicants and military spouses. Check the current general application instructions for the remaining information and submission requirements.
What does New Hampshire doula renewal require?
Plc 1905 requires a birth attended during the certificate term and a perinatal visit in each year of that term. Maintain the underlying evidence while practicing rather than trying to reconstruct it at renewal. Use the actual certificate expiration and current renewal instructions.
An agency can give the worker a preparation date and a named reviewer. Preserve the original certificate and approval history. A pending renewal and an active renewed certificate should have different statuses.
Build renewal tracking around actual practice
Under Plc 1905.01, doula renewal depends on a birth attended during the certificate term and a perinatal visit in each year of that term. Those are different checks. An agency can maintain a term-level birth record and a year-by-year visit review so that a busy period in one year does not conceal a gap in another.
The chapter’s ethical rules also address privacy, personal autonomy and supporting the client’s informed choices. In a shared case, discuss what information the receiving doula needs and follow the applicable permission or disclosure rule. A renewal evidence request should not become an uncontrolled copy of the family’s full history.
How should an agency use the eight-visit framework?
The approved NH-25-0015 amendment describes prenatal/postpartum visits, birth support, practitioner recommendation and additional authorized visits. Track the member’s combined service history rather than creating a fresh allowance for each doula.
For a fictional transfer after prenatal support, the receiving agency confirms prior visits, recommendation and next postpartum contact. It preserves the original worker’s services and documents only its own delivered support. Additional-care requests retain the medical necessity, decision and dates.

Eight visits is not a ready-made claim format
The approved amendment includes a combined allowance of eight prenatal or postpartum visits, with medically necessary extensions through prior authorization. Its payment section separately says doula services are billed once per pregnancy and that twins or other multiple births do not generate additional reimbursement. Read that payment language with the current fee schedule and submission instructions before deciding how an encounter should appear on a claim.
For a fictional pregnancy involving twins and two covering doulas, retain each actual contact and worker while checking the single episode’s visit history and payment arrangement. Do not multiply a package by the number of babies or workers. Confirm the payer’s method for shared care and additional authorized services rather than deriving it from a calendar or the number eight alone.
What support can a New Hampshire member ask for after pregnancy loss?
New Hampshire’s Medicaid statute expressly includes doula support following pregnancy loss for eligible members. The same provision includes support during labor and birth, prenatal and postpartum education, breastfeeding and lactation support, and parenting education. It describes services from a state-certified, highly qualified doula. RSA 126-A:99, subdivisions I and II
For example, a member may want a conversation about support after a loss without enrolling in a parenting class. Ask which support the person wants and whether they would like help contacting another provider. The scheduling team can then verify the doula’s enrollment, the plan’s visit instructions and any required recommendation before quoting a cost. A statutory service category alone does not establish a separate payable encounter for every conversation.
What should a New Hampshire doula note explain?
Record the participant, rendering worker, date, setting or format, actual support, relevant time, response, resources and next action. Link qualification, recommendation and authorization evidence under current instructions. A calendar entry does not establish that a covered service occurred.
Before submission, match the note to the service category, provider approval and dated payment instruction. Keep returned requests and corrections traceable. Do not rewrite a care note to fit a remaining allowance.
What should New Hampshire doula software demonstrate?
Test certification and enrollment as separate approvals, shared visit history and an additional-service request. Inspect the original authorship, approval dates and final payer response. Confirm current OPLC, Medicaid and claim connections individually.

What did CMS approve for New Hampshire doula care?
CMS approved NH-25-0015 on July 8, 2026, effective October 1, 2025. The amendment describes up to eight prenatal or postpartum visits per pregnancy, with additional visits possible through prior authorization based on medical necessity. It requires a qualified practitioner recommendation and Office of Professional Licensure and Certification approval before reimbursement.
The state-plan effective date is distinct from the doula’s actual enrollment effective date and the payer’s submission instructions. Preserve all three. Do not backdate a provider’s approval merely because the benefit’s approved start date is earlier.
What should a New Hampshire agency verify before billing?
| Record | Evidence to preserve | What it answers |
|---|---|---|
| OPLC certification | Approval, pathway and applicable dates | Is the worker certified? |
| Medicaid enrollment | Actual provider approval and identifiers | Is the provider enrolled for this date? |
| Recommendation | Practitioner evidence and service relationship | What supports preventive services? |
| Visit history | Prenatal and postpartum use across workers | What has already been delivered? |
| Additional services | Medical necessity request and authorization | What extended care is approved? |
Use the actual certification and Medicaid enrollment decisions as the basis for readiness. An NPI identifies a provider but does not establish eligibility for every service. For a group, keep the individual rendering doula and organization’s billing arrangement visible rather than storing only the organization’s name.
