Nevada CHW programs must distinguish professional certification from Medicaid's accepted qualification and enrollment rules. A 2026 approved amendment broadened Medicaid-recognized certification beyond a single issuer. This guide explains that change, Nevada Certification Board pathways and the workforce, documentation and referral records program leaders should manage before treating a service as payable. [1,2]

What does a community health worker do in Nevada?
NCB describes CHWs as trusted community-connected workers who help people access health and social services. Its competency materials address the skills needed to support communication, education, navigation and participant goals. [2,5]
Employers should define the population, languages, service area, referral partners and supervisor. A CHW may support people with substantial health and social needs, but the role should not imply authority to perform clinical work requiring another license.
What changed in Nevada Medicaid CHW qualifications in 2026?
CMS approved Nevada SPA 26-0004 on May 15, 2026, effective May 1. It removes the Nevada Certification Board as the sole required certification source and accepts certifications from Nevada Medicaid-approved CHW training programs. [1]
This is a material change for hiring and program design. Do not tell applicants that one issuer is the only Medicaid-recognized route. Equally, do not treat every course as accepted; confirm approval of the specific program and keep the actual qualification evidence.

How are NCB CHW I and CHW II different?
NCB maintains CHW I and CHW II pathways. Both use approved foundational training and ethics requirements. Its current page requires living or working in Nevada at least 51% of the time. CHW II adds diploma or equivalent, 44 additional training hours and experience requirements. [3]
The current CHW II experience structure identifies 450 relevant hours plus 50 closely supervised hours, with specific core-role coverage. Use the current Board page and application rather than an older checklist when planning the evidence. [3]
What additional education does NCB CHW II require?
The current requirements total 44 additional hours: eight behavioral health, six cultural competence or ethics, 20 general CHW learning, eight health equity or community organizing, one HIPAA and one self-care hour. Initial training must fall within the stated five-year window. [3]
Ask NCB how a course is categorized before assuming one certificate satisfies several requirements. Keep the provider, completion date, approved hours and subject area. Employer learning plans should show any remaining category rather than only a total.
How should CHW II supervised experience be demonstrated?
NCB distinguishes 450 hours of relevant work from 50 hours of closely supervised, supervisor-confirmed experience. The latter must include at least five hours in each of ten C3 roles and demonstrate the eleven competencies. The employer should organize the actual activities and supervisor confirmation around that requirement rather than recording only an undifferentiated total. NCB current requirements.
For a fictional experienced worker, map completed activities to roles, identify gaps and ask the supervisor to verify what was actually observed or supported. Do not turn the foundation course, an internal staff review and the credential issuer’s acceptance into a single automatic approval.
Where can Nevada CHWs find foundation training?
Use the current NCB foundation-training list for its credential and the Medicaid-approved program information for Medicaid eligibility. Those approval questions should be checked for the pathway actually being used. [1,3]
Compare current cohort availability, cost, delivery format, language and fieldwork. Ask which award the provider issues and whether there is a separate application afterward. A course that teaches useful skills may still have a different recognition status from the program a payer accepts.
How much does NCB certification cost?
NCB's current overview lists $75 for CHW I application, $150 for CHW II application and $50 for renewal of either credential. It describes a $50 late fee when renewal has been expired for 30 days. Training tuition and other expenses are separate. [2]
Before an employer promises full support, specify which expenses are covered and whether learning time is paid. Confirm the current fee at submission and do not equate an NCB charge with every possible Medicaid qualification route.
How does NCB renewal work?
The current requirements page calls for 20 continuing education hours every two years, including four in cultural competency, humility or sensitivity. Learning must be completed within the renewal period. [3]
NCB's training resources explain acceptable completion evidence, including the organization, activity title, date, participant and hours. Its continuing education page provides training leads. Keep the issued credential and renewal decision separate from an internal education tracker. [6,7]
Can any relevant college course count toward NCB renewal?
NCB’s training-resource page says college coursework is not accepted for its foundational or continuing education requirements. For CE, it requests attendance or participation evidence from an approved organization showing sponsor, activity, dates, participant and hours. Confirm a proposed activity’s recognition before paying for it or assigning it to a renewal category. NCB training resources.
These are NCB’s credential-maintenance rules. The May 2026 Medicaid amendment’s broader acceptance of approved training programs should not be rewritten as a change to every NCB renewal rule.
Can a CHW apply after training and immediately bill Medicaid?
Training completion alone does not establish an active billing arrangement. Use Nevada Medicaid's current Provider Type 89 instructions and enrollment process to verify the worker, supervising relationship and responsible billing provider. [4]
Document the effective dates and service setting before the first claim. A software account, NPI or submitted application should not be treated as proof that the relevant enrollment and payer conditions are complete.
What does the approved plan say about covered CHW work?
The approved plan includes navigation to healthcare, identifying needs, health education, resource information and referrals, connections with preventive or community services, and health literacy. It requires an appropriate licensed-practitioner recommendation and documentation in the recipient's medical record. [1]
For a program leader, connect the activity to the actual covered service. Helping a participant identify a resource is different from the resource itself being a separately reimbursable benefit. Preserve what the CHW did and which need it addressed.
How should programs handle service limits and authorization?
Use the current Medicaid guide, manual and applicable plan instructions for time units, limits, groups and authorization. The approved plan permits medically necessary exceptions to service limitations with prior authorization. An exception process should not be treated as unlimited routine coverage. [1,4]
Before configuring software, have the responsible billing team confirm the current rules in writing. Test the edge of a limit and what happens when authorization is pending. Do not copy another state's time units into Nevada templates.
What are the current Provider Type 89 time and supervision rules?
The Nevada Medicaid billing guide updated July 27, 2026 lists individual and group self-management education in 30-minute units. It limits an individual to four units per day and 24 units per calendar month, equivalent to two daily and twelve monthly hours. Prior authorization is not needed within those listed limits; exceeding them requires authorization. Payment still depends on eligibility and the other benefit conditions. Provider Type 89 billing guide, page 1.
The guide requires supervision by one of its listed enrolled practitioners, including physicians, APRNs, physician assistants, dentists and specified behavioral health practitioners. It separately directs telehealth questions to the telehealth chapter and billing instructions. The appearance of an originating-site code does not mean every remote contact automatically qualifies as a payable CHW service. Current PT89 guide.
For a fictional group session, confirm the actual number of recipients, each recipient’s need and time support. Maintain the licensed recommendation in the medical record, as the approved state plan requires, and identify the responsible billing and supervising arrangement. A group invitation alone does not establish service attendance. Approved Nevada plan, CHW services.
Are Tribal CHRs addressed separately?
Yes. The approved amendment includes CHR services in an IHS or Tribal Health Clinic context and identifies completion of IHS CHR training. That pathway should be reviewed on its own terms rather than forced into an ordinary NCB application checklist. [1]
Keep the worker's actual role, training, clinic relationship and service records clear. A program serving several settings should not assume that one enrollment arrangement applies to every organization.
What should Nevada CHW documentation capture?
Build a note around the participant's goal, service provided, response and next action. Include the date, worker, contact method and time support required by the applicable service. Preserve recommendation, authorization and supervising relationships where relevant.
For a referral, distinguish offered, accepted, sent, scheduled and completed. A partner without capacity needs a different follow-up plan from a completed connection. Keep participant choice and unresolved needs visible.
What should Nevada CHW software demonstrate?
Test the 2026 qualification change directly. Use one fictional worker with NCB evidence and another with a different accepted Medicaid-approved program. Ask whether the workflow can preserve the actual issuer and evidence rather than forcing both into a single credential label.

Which reports help Nevada program leaders?
Separate credential readiness, Medicaid participation, participants served, contacts and confirmed outcomes. Keep workforce evidence effective for a service date rather than reporting only today's status.
Trace a report total to its underlying records and test a corrected note, an expired credential and an unresolved referral. Compare the full cost of access, setup, migration, training and support with the work the program needs to complete.
Frequently asked questions
Is NCB the only Medicaid-recognized certification source?
No. The approved 2026 amendment broadens accepted certification to Nevada Medicaid-approved CHW training programs. [1]
Are CHW I and CHW II the same credential?
No. NCB publishes distinct pathways and CHW II has additional conditions. [2,3]
Can an old CHW II checklist be used without checking updates?
Use the current requirements and application. Published training categories and other details have changed over time. [3]
Does a completed course guarantee payment?
No. Enrollment, provider relationships, covered services, documentation and payer conditions still apply.
What should a Nevada CHW software demo show?
Test qualification evidence, enrollment status, a supported service record, referral outcomes and a report traced to source records.
