New Hampshire has an active voluntary CHW certification system through the Office of Professional Licensure and Certification. The law protects the certified CHW designation without requiring certification for every CHW job. Current rules include training and experience pathways. This guide explains the distinction and the evidence program leaders should manage. [1,2]

What does a CHW do in New Hampshire?
New Hampshire's law describes public health work grounded in understanding a population's experience, language and culture. It includes education, social support, community health integration, access to services and screenings within the authorized role. [1]
A local job description should explain the actual population, languages, referral partners, supervisor and escalation process. A broad CHW role does not authorize work requiring another professional license. Employers should preserve the worker's community relationships when adding reporting and productivity expectations.
Is CHW certification required for employment?
RSA 326-N describes certification as voluntary and says it is not required for purposes of employment. It separately protects the certified CHW title and CCHW designation. Employers may still set qualifications for particular positions. [1]
State clearly whether certification is required for the specific role, preferred or part of a development plan. Do not treat a training enrollment as an issued credential or label an uncertified worker CCHW in a public directory.
When did the certification process begin?
DHHS announced that voluntary certification became available April 1, 2025. Older state summaries describing certification as only proposed predate that operational announcement. [2]
For an applicant, the practical next step is the current OPLC process. Keep the application, supporting evidence, confirmation and issued decision. An internal staff onboarding checklist should show progress without implying that the state has approved an incomplete application.
What are the training and experience pathways?
The current rules allow a training route or 4,000 hours of qualifying paid or unpaid CHW work within five years. Only time performing qualifying duties counts. Applicants also need the required education evidence. [3]
Plc 2003.05 requests each organization’s name and address, the months and years worked, qualifying hours and duties, and a verifier’s name, daytime telephone number and role. The qualifying work described in Plc 2003.02 includes education, coordination, informal support, advocacy, assessment, capacity building, disease prevention and outreach. Count the actual qualifying work rather than every hour associated with a healthcare job. Plc 2003.02 and 2003.05.
For example, a fictional worker has 4,200 total hours in a mixed administrative and outreach position, but 3,100 involved qualifying CHW duties. The total employment history does not automatically meet the 4,000-hour route. A useful verification worksheet separates each duty, dates, qualifying hours and the person able to confirm them. This example illustrates the counting issue, rather than an OPLC eligibility decision.
Which training organizations are recognized?
DHHS's certification announcement directs learners to approved training. Southern New Hampshire AHEC publishes core training information, while White Mountains Community College describes OPLC recognition of its relevant education program. Check the current offering and approval for the course you intend to use. [2,5,6]
Compare delivery method, dates, cost, attendance, assignments and completion evidence. A provider may offer several courses with different purposes. A short professional development workshop should not be assumed to replace foundational training.
What changed in the training rule in September 2026?
The current qualifying-training rule names North Country Health Consortium, Southern New Hampshire AHEC and White Mountains Community College, and addresses alternative curricula and waiver criteria. The amendment is effective September 21, 2026. [3]
Plc 2003.03 also addresses other curricula meeting its competency criteria. Its alternative-training waiver references C3 standards or acceptance by at least ten other U.S. jurisdictions that certify CHWs, through the rule’s waiver process. This does not make every out-of-state credential automatically transferable. Supply the curriculum, official transcript and other evidence OPLC requests for the actual route. Plc 2003.03.
Training-provider hours and admissions rules also need careful reading. Southern New Hampshire AHEC describes a 56-hour core course, while White Mountains Community College identifies OPLC recognition within its medical-assistant education program. The college’s admission, health and placement conditions belong to that program. They should not be presented as universal OPLC certification requirements. SNHAHEC course information, WMCC program information.
How do you apply to OPLC?
Follow the current OPLC CHW instructions and profession-specific supplement. The rule requires training or experience evidence and says no criminal background check is required for this application. Background and character disclosures still need truthful responses. [3]
For the training route, Plc 2003.05 requests the training organization’s contact details, attendance dates and completion certificate or equivalent documentation. For the experience route, prepare the detailed organizational verification instead. Answer character and disciplinary questions accurately even though the CHW rule does not require a criminal background check. A school’s separate placement screening does not change that OPLC rule. Plc 2003.04 to 2003.05.
Before submitting, open every attachment and retain a copy. If the office requests clarification, respond through its official process. Do not send a participant record as workforce experience evidence when a suitable employer verification can establish the work.
What does certification cost?
The current fee rule, Plc 1002.54, lists $160 for initial certification, renewal or reinstatement after expiration, with a two-year term. The CHW rules provide an active military and military-spouse fee exception. Training tuition and other expenses are separate. [3,4]
An employer paying for certification should clarify which costs and learning time it covers. Avoid using an older legislative fiscal estimate as the current application fee.
What continuing education is needed for renewal?
The rules require at least six credits during the two-year period before renewal, through permitted live or self-directed learning. Renewal records identify the activity and include an attestation. [3]
Keep the sponsor, date, length, instructor and location with the learning record. The CHW rule requires the renewal attestation and incorporates the general evidence and audit provisions. Those provisions require evidence for the current and most recently renewed terms, or longer when another applicable requirement demands it. Confirm the actual activity’s acceptance rather than assuming all workplace meetings earn credit. Plc 2004.03 to 2004.04, Plc 308.04.
What happens when certification approaches expiration?
The rule makes the certificant responsible for knowing the expiration and applying before it. Use OPLC's current renewal or post-expiration process for the actual status. General renewal rules describe the ordinary two-year renewal term. [3,7]
The general renewal rule opens submission no more than 60 days before expiration and requires receipt by expiration. The CHW rule provides for a 60-day notice, but the worker should contact OPLC if it is missing. Keep learning evidence, the submitted renewal and the agency’s issued status as separate records. Plc 308.05, Plc 2005.02 to 2005.03.
An expired certificate must not be represented as active. Plc 2005.07 distinguishes expiration, suspension or revocation under the general restoration rules from a lapsed credential, which requires a new application under Plc 2003.04. Ask OPLC which process applies to the recorded status before simply paying a renewal fee. Plc 2005.07.
Can certified CHWs provide telemedicine?
RSA 326-N:7 permits certified CHWs to provide services through telemedicine. That professional authority does not by itself establish Medicaid or commercial payer coverage, an allowable billing code or payment for every remote contact. [1]
For implementation, confirm the service agreement, privacy process, participant access and documentation expectations. Distinguish a telephone outreach attempt from a completed service under the actual program.
Does New Hampshire certification guarantee Medicaid reimbursement?
No. Certification establishes a workforce credential, while payment requires an applicable benefit or contract. Confirm the applicable payer benefit and organization agreement before forecasting reimbursement.
Before building a reimbursement forecast, identify the payer, participating organization, eligible population, service and required records. Keep grant-funded, employer-funded and contracted work accurately attributed rather than treating every CHW contact as a claim.

What should New Hampshire CHW documentation show?
As a program-design starting point, document the participant's goal, identified barrier, support provided, response and next action. Include date, worker, contact method and follow-up owner. Apply time and review fields where the actual arrangement requires them.
A referral can be offered, accepted, sent, waitlisted or completed. Preserve the actual result and participant choice. A completed staff task should not automatically close a need that remains unresolved.
What should CHW software demonstrate for New Hampshire teams?
Test a worker using the experience route and another using training evidence. Keep application status and the issued credential separate. Then follow a fictional participant through a contact, referral and supervisor review.

Which reports help program leaders?
Separate qualified staff, pending applications, upcoming renewals, people served, contacts and confirmed outcomes. Define each measure before combining data across partner organizations.
Trace a total to its source records and test a corrected note or expired credential. Reports should explain unresolved barriers as well as completed work. A high contact count alone does not establish better outcomes or payable services.
