Wyoming CHW program leaders can use the University of Wyoming’s training resources and state rural-health materials to plan workforce development. Training completion does not by itself establish payer eligibility. Older clinic billing guidance also needs careful updating, especially when a 2024 handout still describes Medicare’s former G0511 reporting approach. What does a CHW do in Wyoming?
The University of Wyoming describes CHWs helping community members navigate health care, support health-related change and work toward wellness. A program should define the actual population, service area and responsibilities rather than treating every outreach or navigation role as identical. University of Wyoming training
For a rural or frontier team, access barriers may affect how staff plan contacts and follow-up. Build a process that preserves the participant’s priorities, makes the next action clear and gives workers someone to consult when needs exceed the role.
Wyoming CHW training options
The University of Wyoming offers a CHW continuing-education program through a separate online learning system. Its page describes four mini-courses and competencies in navigation, professional practice, communication, community health and related areas. It is a program-specific education offering, not proof of an individual Medicaid enrollment. University of Wyoming training
UW’s program description retains historical scheduling language about January 2024. Use the university application contact to confirm an upcoming start, course sequence, admission and available financial support. University of Wyoming training
Training costs and assistance
The university page describes grant support for Wyoming residents and directs nonresidents to discuss costs. Confirm whether support remains available for the actual cohort and what expenses it covers before promising a free place to a worker. University of Wyoming training
Budget for staff participation time, equipment, connectivity and coverage of current responsibilities as well as tuition. An education grant may support the course while leaving the employer responsible for wages or other participation costs.
Questions to ask before selecting a course
- What competencies and practical skills will the worker demonstrate?
- What attendance, assignment and assessment requirements apply?
- What are the current course dates and completion expectations?
- Is assistance available for this applicant and cohort?
- What completion evidence is issued?
- Does the employer or funder recognize the course for the proposed role?
Obtain answers before setting an employment deadline. A prior grant award or historical course announcement does not establish an open seat.
Certification and job readiness
UW describes a continuing-education program and its completion process. An employer should decide which training it accepts for a given job. Verify any separate payer-specific qualification with that payer. University of Wyoming training Wyoming Medicaid
Do not borrow Wyoming’s behavioral-health provider certification or HCBS case-manager requirements simply because those programs also serve community members. They concern different services and actors. A CHW course should not be relabeled as one of those approvals.
Build an employer onboarding process
A worker should know the organization’s population, local resources, referral partners, documentation expectations and escalation contacts. Provide practice with a participant conversation, a referral and a clear note, followed by feedback.
Keep onboarding evidence separate from course completion. A class may provide broad skills while the employer must explain its own procedures. Record what the worker demonstrated and what support remains rather than using one unexplained “ready” checkbox.
Why the 2024 sustainability handout needs updating
The Wyoming-hosted January 2024 handout discusses Medicare opportunities for RHCs and FQHCs, including general care-management reporting under G0511. It is useful historical context, but its code and payment discussion should not be reproduced as current 2026 billing instructions. Wyoming 2024 financing handout
CMS’s later claims-processing update says clinics could continue G0511 only through September 30, 2025 under the transition, after which they must use the applicable individual codes. The same update addresses designated care-management services for 2026. CMS clinic claims update

Medicare guidance is different from Wyoming Medicaid coverage
CMS’s Medicare Community Health Integration (CHI) guidance covers a specific service under the federal Physician Fee Schedule. It requires a qualifying initiating visit at which the billing practitioner identifies an unmet social need that significantly limits their ability to diagnose or treat the medical problem addressed. Advance patient consent may be written or verbal and must be documented. Auxiliary personnel, which may include a CHW, must meet the applicable incident-to requirements. CMS CHI questions CMS CHI coverage manual
For Wyoming Medicaid, use the state’s provider manual and service-specific payer instructions. Identify the member’s coverage on the service date, the enrolled billing provider, the service and applicable payment terms. The federal Medicare guidance cannot establish a corresponding Medicaid benefit. If your organization is paid under a grant, identify that project and its reporting conditions separately. Planning clinic-based CHW work
For a clinic arrangement, identify the responsible practitioner, the service being supported, participant eligibility and how the work is documented. Confirm current code, time, consent, supervision and nonduplication requirements with the billing team before configuration.
A software platform can help organize the evidence, but it cannot create coverage or guarantee a claim outcome. Ask the vendor to demonstrate the documentation and reporting workflow your organization uses rather than relying on a generic billing label.
Funding a Wyoming CHW program
Wyoming’s Rural Health Transformation Program application proposed county-level care coordination for high-risk people with chronic conditions. In its proposed selection criteria, the state prioritized models using community health workers or community EMS, cooperation among providers, and technology to bring care closer to home. This describes the state’s funding design. It does not supply a Medicaid fee or certify an individual practitioner. Wyoming rural application
For an actual placement, use the Department of Health’s separate year-one award summaries and the named recipient’s agreement. Ask who employs the worker, which county the award serves, whether the work is within the grant period and which activities the grant requires. Compare that answer with the university’s course requirements and the relevant payer before promising a funded assignment. Wyoming rural program hub University of Wyoming training

For an open referral, distinguish attempts to reach the person, their acceptance of help and the result they report. Decide who will follow up when travel remains a barrier.
Document the participant’s goal and next action
A useful record connects the participant’s need with the contact, assistance, response and follow-up. Identify the worker and date. Preserve referral history so another team member can continue the work without repeating the intake.
For example, a participant may need help arranging a follow-up appointment. The CHW discusses the barrier, identifies options and agrees on a next step. If distance or scheduling prevents access, retain that result and the alternative considered. A referral sent is different from an appointment attended.
A Wyoming clinic checks a rural referral before assigning payment
A participant tells a rural clinic they cannot travel for a follow-up appointment. The CHW can ask what assistance the participant wants and help identify a reachable option. If billing Medicare CHI is under consideration, the billing practitioner must establish how the unmet need affects diagnosis or treatment in the qualifying visit. Record the participant’s advance consent, the person furnishing the subsequent activity and its actual duration as required by CMS. CMS CHI questions CMS CHI coverage manual
A second person might have the same transportation barrier but be served through a local project or Wyoming Medicaid. Verify that specific arrangement. A ride request, a CHW training certificate or a Medicare code in an old file cannot establish which entity pays. If the participant declines the suggested option, record that choice and an agreed follow-up without reporting a completed connection.
