What are Illinois’s doula training and legacy requirements?

The SIU doula guide describes a training route with program verification and experience at three births in the preceding 12 months, either as lead doula or shadowing a lead. Its legacy route documents support for five clients across prenatal, labor and delivery, and postpartum care in the preceding three years.

The current SIU pathway page continues to list training and legacy options. Its training checklist describes three perinatal doula experiences in 12 months and approved labor/postpartum training; the legacy checklist describes five perinatal experiences in three years. Both list age 18 and the specified CPR or BLS, HIPAA, cultural competency or bias, and trauma-informed training. The legacy checklist separately lists pregnancy and birth anatomy and physiology. Use the actual pathway’s application evidence rather than assuming a private course title proves every requirement.

The older guide has more detailed birth and verification wording, permits review of a non-preapproved course with a syllabus, and describes temporary provisions for early applicants. It also calls the legacy option time limited. Because the live page still lists that option without a deadline, obtain SIU’s current acceptance and verification instructions before relying on an early-applicant exception or a presumed closing date. Do not infer that completing this older checklist guarantees certification.

What is the practical enrollment sequence?

Step 1: Complete SIU certification

Choose the supported route, obtain verification from the relevant program or references, submit the application and retain the decision. SIU certification confirms that the doula meets the Illinois Medicaid program’s qualifications. A separate private course does not replace it.

Step 2: Select the correct IMPACT enrollment type

The HFS enrollment notice distinguishes rendering/servicing enrollment from an individual or sole-proprietor arrangement. Choose the role that matches who will submit claims. Keep the certification number and dates with the actual approval and organizational affiliation.

Step 3: Establish the payer relationship

For a managed care member, confirm the applicable MCO process. For fee-for-service, follow current professional claim instructions. Save the submission route, billing contact and effective dates. Do not mark all payers ready when one enrollment step is complete.

How does Illinois’s standing recommendation affect intake?

HFS describes the standing recommendation in its annual program report. It allows eligible members to access covered doula services without obtaining an individual physician referral for that purpose. The program still needs a service record and the other conditions of coverage.

Record the recommendation basis in the note or linked episode record under current instructions. Do not turn the standing recommendation into a claim that all possible services, durations or codes are covered. Eligibility, provider approval and the service delivered still require review.

Can an Illinois Medicaid doula accompany a member in the birth facility?

The October 2025 HFS facility notice addresses hospitals with licensed obstetric beds and birthing centers. It says their policies must allow the member’s Illinois Medicaid-certified and enrolled doula to accompany the member before, during and after labor and childbirth, without counting the doula against the support-person quota.

Before attendance, confirm facility coordination procedures and how the worker identifies their approved status. Keep participant preferences and backup arrangements clear. Facility access and reimbursement are distinct issues; permission to accompany the member does not establish the payment code for an encounter.

How should Illinois agencies manage renewal and affiliation changes?

Maintain a calendar for the three-year certification and submit recertification evidence through the required channels. When a doula changes organizations, preserve the affiliation that applied to earlier services and establish the correct future billing relationship. An organizational reassignment should not rewrite prior authorship.

Use a fictional case to test the workflow: a certified worker delivers prenatal support, changes groups, and later supplies postpartum care. A reviewer should be able to identify the rendering worker, billing entity, recommendation basis and applicable rate for each service date.

What should Illinois doula software demonstrate?

Follow one service from SIU qualification and IMPACT approval through note review, professional submission and response. Ask which MEDI, 837P, MCO or other billing tasks are supported today and which require another process. Software should make the enrollment evidence available, not imply that certification alone approves payment.

What makes an Illinois doula eligible for Medicaid reimbursement?

HFS’s doula enrollment notice requires certification through the Illinois Medicaid-Certified Doula Program administered by Southern Illinois University before IMPACT enrollment. Certification lasts three years. Rendering providers associate with a billing group or organization; sole proprietors have a different enrollment option. Preserve the actual enrollment decision and affiliations, not just a training certificate.

Treat these approvals as dated records. A certificate expiring next month, a pending IMPACT application and an approved plan relationship require different follow-up. Give the scheduler a usable view of who is approved for the proposed service and the billing team access to the underlying evidence.

How should an agency organize Illinois doula billing?

RecommendationStanding or individual recommendation usedIs the basis documented in the service record?
EncounterService, date, worker and relevant timeDoes the note support the selected service?
Rate versionSchedule and service-date effective periodWas the applicable rate used?

HFS requires the documentation to identify whether the statewide standing recommendation or an individual recommendation was used. The notice distinguishes that documentation from claim information. It also describes 837P electronic claims and MEDI entry. Keep care evidence and submission data connected without copying every note into a claim.

Where should Illinois doula programs find current rates?

Use HFS’s doula fee schedule page. The current index labels the latest May 1, 2026 schedule as updated October 14; the linked PDF itself is marked updated October 15, 2026. Preserve the actual linked document and its effective date, rather than relying only on the index label. An older onboarding slide should not be the agency’s rate authority.

Save the schedule version used for each payment configuration and check procedure-specific limits and footnotes. Do not convert a set of maximum allowable payments into a guaranteed package total. For managed care, reconcile the current plan instructions and agreement as well.

Which Illinois doula billing units and rates apply?

The latest linked HFS fee schedule is effective May 1, 2026 and marked updated October 15. Its education and pregnancy-loss support entries use 15-minute units. The following are published FFS schedule amounts, subject to coverage, medical necessity and claim rules; a listed code does not guarantee payment.

ServiceCodePublished unit amountListed daily limit
Individual prenatal educationS9445$15 per 15 minutes8 units
Individual postpartum education and resource supportS9444$15 per 15 minutes16 units
Group prenatal or postpartum educationS9445 or S9444 with HQ$5 per 15 minutes per member8 units
Support during or after miscarriage or abortionT1032$15 per 15 minutes8 units
Appropriate labor and delivery support category59409, 59514, 59612 or 59620$7201 listed unit

HFS’s May 2026 limit notice applies the education and pregnancy-loss daily limits to FFS and HealthChoice Illinois. It also requires each covered encounter’s start and end time in hours and minutes. Recording a general visit duration without those endpoints is not the complete time documentation described in that notice.

What changes when prenatal education happens during a hospital stay?

The fee schedule includes a specific inpatient condition for S9445: separate prenatal education billing alongside labor and delivery support requires a clinical indication for a medically necessary prolonged antepartum hospital stay, supported in the patient's medical record. Routine presence in the hospital does not by itself establish that exception. Keep the reason and supporting medical documentation available for review rather than assuming every hospital contact is separately payable. HFS fee schedule, S9445 inpatient note

These are daily limits, not a state-published total pregnancy package. Do not add an arbitrary number of maximum days to advertise a guaranteed episode payment. For a fictional 60-minute covered individual prenatal education encounter, four 15-minute units at the published $15 amount equal $60 before claim review. That arithmetic does not establish eligibility, medical necessity or successful payment.

How are postpartum appointment attendance and education different?

Code 59430 concerns accompanying the birthing person to a practitioner postpartum appointment, rather than an ordinary doula education visit. HFS permits one attendance payment for the period within 26 days after delivery and one for days 27 through 89, subject to the requirements. The schedule lists $50 for each qualifying attendance and instructs providers to report the actual delivery date in 837P Loop 2300, DTP*454 (Initial Treatment Date). It pays only the first payable claim received for each attendance period. A duplicate submission within the same period does not create another allowance. HFS fee schedule, attendance footnote

Code 99381 concerns facilitation and attendance at the first newborn practitioner visit within two weeks after delivery. Keep the newborn appointment, the participant’s postpartum practitioner appointment and the doula’s own education encounter distinct in the record. Use the actual service and current payer instructions when choosing a code; the participant attending a medical visit alone does not document doula attendance.

Which services can use telehealth?

The enrollment and billing notice permits telehealth for S9444, S9445 and T1032 using the applicable GT audio/video or 93 audio-only modifier and place of service 02 or 10. That is not a general telehealth permission for every labor or appointment-attendance code. The fee schedule also says the amount and nature of telehealth support must meet the key components of a face-to-face encounter. HFS telehealth instructions

A practical note can identify the service, date, start and end times, rendering doula, communication method, participant location as needed for claim selection, support delivered and recommendation basis. Confirm the actual plan’s submission requirements. Preserve an attempted phone call separately from a completed covered support encounter.

Frequently asked questions

Does a private doula certificate replace SIU certification?

No. HFS requires the Illinois Medicaid certification before IMPACT enrollment.

Does SIU charge for this certification?

HFS says SIU will not charge doulas a certification fee. Training costs are separate.

How long is the credential valid?

Three years, followed by recertification and submission of proof to HFS.

Can experience support certification?

Yes. SIU provides a legacy pathway with specific verification requirements.

Does rendering-only enrollment allow direct state billing?

That enrollment is for an individual rendering services while another billing entity submits claims.

Must every member obtain an individual physician referral?

Illinois’s standing recommendation removes that individual-referral step for covered services. Document the basis under current instructions.

Does facility access prove an encounter is payable?

No. Coverage and billing conditions still apply.

Where should an agency obtain current rates?

Use the dated HFS doula fee schedule and applicable managed care agreement.
