Indiana doula programs must distinguish birth support from the separately defined Indiana Health Coverage Programs community health worker service. A worker can hold more than one qualification, but the activity and provider arrangement determine the payment route. This guide explains that boundary, training decisions, funding agreements, intake and a practical review process for agencies using several staff roles.
What should you know at a glance?
| Topic | State-specific answer |
|---|---|
| Key distinction | Doula support and IHCP community health worker services are separate roles |
| CHW authority | Use the current IHCP bulletin and provider instructions |
| Funding decision | Confirm the actual benefit or service agreement |
| Training record | Identify the credential, issuer and role it supports |
| Private payment | Use an agreed service package and receipt process |
| Community contract | Retain eligibility, scope, reporting and payment terms |
| Service note | Describe actual activities instead of relabeling a package |
| Agency priority | Accurate role assignment and care continuity |

Does the Indiana CHW pathway cover every doula activity?
No. IHCP Bulletin BT2024198 defines community health worker services and qualifications separately. It discusses diagnosis-related education in conjunction with a healthcare team and cultural brokering between members and providers. Those descriptions do not turn an entire birth-support package into CHW care.
Before using that route, verify the worker’s CHW qualification, the eligible provider arrangement, the activity, any ordering or supervision condition and current claim instructions. Document what actually happened. A doula credential alone does not establish all of those elements.
What provider arrangement does the CHW bulletin require?
The bulletin says CHWs do not independently enroll with IHCP. They work for an enrolled billing provider under an eligible enrolled supervisor, and the billing provider retains certification evidence. It excludes insurance-navigation assistance, case management, care coordination and arranging or providing transport from the described CHW payment. Those exclusions matter when a doula package includes similar practical help. IHCP CHW bulletin, pages 2 and 3
For a fictional worker with both qualifications, a prenatal conversation may include emotional birth preparation, help arranging transportation and a separately defined education activity. The agency should describe each accurately and check the relevant service conditions. It should not charge every minute to CHW merely because one portion involved education. The doula service agreement can still explain which nonbillable coordination the agency has promised to provide.
How should an Indiana doula choose training?
Start with the work you intend to provide and the organization or funder you intend to serve. Compare birth and postpartum course content, practical experience, mentorship, final certification and renewal. If you also want to work as a CHW, obtain that role’s current requirements separately rather than assuming a doula course satisfies them.
An agency should keep separate evidence for each role. Record course completion, final credential, continuing education and organizational onboarding. A worker with several qualifications needs a clear assignment for each encounter so notes and reports do not obscure which service was delivered.
How should Indiana agencies verify a proposed Medicaid-funded service?
Indiana’s member coverage guidance directs questions about specific services to the provider or health plan. Request written confirmation of the actual benefit, eligible provider type, referral or approval requirements and payment route. General maternity coverage is not enough to establish a doula claim.
Do the published stakeholder recommendations create a registry or payment pathway?
FSSA's July 19, 2024 cover letter publishes findings from its doula stakeholder workgroup. The attached May 2024 recommendations discuss training, a verification body, a registry, Medicaid enrollment and reimbursement. The cover letter identifies them as recommendations. Do not treat the proposed sequence as an operating registry application, an approved qualification rule or a current payment promise. If a course or partner points to that report as proof of Medicaid eligibility, ask for the later implementing policy and the actual enrollment instructions. The workgroup report and the CHW bulletin answer different questions; neither authorizes relabeling birth attendance as CHW education.
Keep the response and its date with the funding record. If no payable route is established, make a separate, clear decision about private, grant or contract support. Do not borrow another state’s doula code or relabel labor attendance to fit a different service definition.

What does a useful dual-role example look like?
In a fictional program, a worker provides a contracted doula birth-support package and also performs an eligible CHW activity through an authorized healthcare arrangement. The records distinguish the assignment, date, actual activity, required evidence and financial handling. The same undifferentiated time is not charged twice.
The review question is specific: does this documented activity meet this service pathway under this provider arrangement? If the answer remains unclear, retain the care account and resolve the payment question separately. A reviewer should not rewrite the service description to make a charge pass.
How should a reviewer separate time and responsibility?
Use a role-specific service record rather than a single undifferentiated pregnancy package. Identify the participant, date, worker, activity, time, relevant supervisor or contracting party and funding decision. A review should be able to answer whether a contact was completed, which role delivered it and which agreement or benefit supports the payment request.
If a supervisor asks for clarification, preserve the actual original account and add the requested detail honestly. A scheduler can correct an assignment or an invoice while the record continues to show the worker who actually provided the support. Do not create a missing education activity after a claim is returned or convert travel into direct support to meet a payment rule.
How should an Indiana doula agency structure private and grant-funded support?
For private support, define included contacts, on-call and backup terms, travel, cancellation and fees. For a grant or partner contract, identify eligibility, award period, authorized capacity, allowable costs and reporting units. A referral becomes a funded case only after the relevant acceptance decision.
At intake, agree on a primary worker, preferred communication, likely birth setting and next contact. Preserve participant permission for partner communication separately from acceptance of the service package. If funding changes, retain the earlier allocation and document the future arrangement.
How should software prevent role and payment confusion?
Test a participant supported by a doula and a CHW, including a worker who holds both qualifications. Ask the system to show the role, service record and funding basis for each contact. Inspect the report for duplicate time or service counts and confirm how corrections preserve authorship.
For daily care, evaluate backup assignment, postpartum reminders and referral follow-through. Confirm role-specific templates, invoice exports and billing functions with the vendor using your actual service definitions.

How are doula and community health worker services different?
IHCP publishes a separate community health worker services bulletin. That pathway has its own service definitions and provider requirements. A person holding more than one qualification does not make every activity interchangeable or allow a birth support package to be relabeled as community health worker work.
For a worker with multiple roles, identify which role applies to each encounter, the activity delivered, applicable qualifications, supervision or other required arrangement and the authorized payment route. Use the current IHCP guidance and plan instructions for that specific service. Keep clinical care and doula support within their respective scopes.
| Encounter category | Evidence to establish | Financial handling |
|---|---|---|
| Private-pay doula support | Agreed package and payment terms | Participant invoice or receipt |
| Grant-funded support | Award eligibility and service definition | Allocate to the correct grant |
| Community health worker activity | Applicable IHCP service and provider requirements | Review under that service pathway |
| Plan-specific doula arrangement | Written benefit and provider approval | Use the confirmed payer process |
What should Indiana agencies capture during intake?
Record the participant’s actual program and plan, preferred support, funding decision and assigned coordinator. If coverage is unconfirmed, create a verification task with an owner and due date. Staff can continue arranging support while accurately explaining what payment remains unresolved.
When a referral arrives from a hospital, distinguish permission to contact the participant from agreement to pay. Record who made the referral, what the participant agreed to and whether a signed funding arrangement exists. A referral source is not automatically the invoice recipient.
Frequently asked questions
Does a doula certificate establish CHW billing eligibility?
No. Use the CHW pathway’s own qualification and provider requirements.
Can the same worker hold both roles?
A worker may hold several qualifications. Each encounter still needs an accurate role and service basis.
Can a whole birth package be relabeled as CHW care?
No. Match each actual activity to the applicable service definition.
Does pregnancy coverage prove doula reimbursement?
No. Confirm the specific benefit and provider pathway.
What should a payer confirmation include?
The benefit, eligible provider, conditions, effective date and submission route.
How should a grant-funded referral be counted?
Use the award’s actual unit and acceptance status, with service records supporting completed work.
Should a payment correction change the original note?
Preserve the original care account and add a traceable correction or clarification.
What should an Indiana software test include?
Two staff roles, separate funding and one report that can be reconciled to the underlying encounters.
