KanCare began covering doula services July 1, 2024. Provider qualification, enrollment and a recommendation from an eligible practitioner are separate steps. This guide explains the coverage framework, recommendation timing and practical records for a Kansas doula agency. Use current KMAP and plan instructions for the actual qualification and billing requirements.

How should Kansas doulas check qualification requirements?
Start with the enrollment and attestation materials linked from the KDHE Doula Toolkit. Before buying training, obtain the current qualification checklist and compare the exact credential, experience evidence and application documents with your own record.
Keep course completion, practical experience, submitted application and actual provider approval as separate records. Ask the enrollment team to clarify an uncertain individual or group arrangement before submitting conflicting identifiers.
What is the practical KanCare enrollment sequence?
Step 1: Prepare the qualification and provider records
Collect the pathway evidence, attestation and current identifying information. Use the KDHE Doula Toolkit to reach current enrollment materials and instructions. Keep the individual NPI distinct from any group identifier.
Step 2: Retain enrollment and payer approval
Save the actual decision, effective date and billing relationship. For managed care work, confirm the applicable plan process and contract. An NPI or completed training file does not establish payer participation.
Step 3: Establish recommendation and service tracking
Before service, identify the written recommendation, applicable standing order or standard referral form supporting care. Link the evidence to the participant’s episode and assign ownership for missing information. Do not treat an unsigned request as completed evidence.

How should a Kansas agency confirm billing rules?
Use the current KMAP doula coverage instructions, fee schedule and applicable plan agreement before configuring codes, modifiers, units or frequency limits. Keep the actual source version and effective date with the billing configuration.
Record prenatal support, birth attendance and postpartum support separately. Preserve actual service time and prior use when a worker changes or another pregnancy begins. A scheduled contact, an actual visit and a billing unit should not be treated as interchangeable.
The approved state plan separately says services for the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) population are furnished based on medical necessity. Refer an exception question through the actual plan or KMAP process and retain the determination.
Can Kansas doulas use telehealth?
The approved Kansas amendment permits prenatal and postpartum telehealth, while labor and delivery support cannot use that route. Confirm current technical, documentation and payer instructions for the actual encounter.
Record the service format accurately. A phone check-in and an eligible telehealth visit are not automatically the same thing. Do not convert between them after the fact solely to fit the payment package.
What does a useful Kansas timed note show?
For a fictional prenatal encounter, the note identifies the participant, rendering doula, date, setting or format, actual service time, support provided, response and next action. The reviewer can connect it to the recommendation and calculate units using the applicable rule.
A shared-care episode should carry cumulative prenatal and postpartum use forward when a backup is assigned. Preserve the first worker’s notes and the second worker’s actual attendance. Assigning a new person does not create a new pregnancy allowance.
How should Kansas doula software support payment review?
Ask for a demonstration that distinguishes minutes, units, visits and birth attendance. Include a recommendation received before care, a worker transfer and separate pregnancy episodes. Inspect how the system flags an exception without deleting or rewriting the care account.
Confirm current KanCare payer connections and claims capabilities separately. A care record can support review even when submission occurs in another system. Keep the submission reference, returned reason and final payment tied to the underlying service.

What does Kansas Medicaid cover for doula care?
The approved Kansas state-plan amendment is effective July 1, 2024. It describes prenatal and postpartum support and one labor and delivery service. Prenatal and postpartum visits can use telehealth; labor and delivery support cannot. Confirm current KMAP and plan instructions for codes, settings, rates and submission.
What recommendation documentation is required?
The KDHE Doula Toolkit identifies the Kansas-licensed practitioners who may recommend services: registered nurses, clinical social workers, nurse practitioners, physician assistants, certified nurse midwives and physicians. A referral coordinator can check the recommender’s actual credential against that list rather than relying on a general job title.
Methods include a written recommendation, an applicable standing order or the standard referral form. KDHE directs doulas to obtain the signed form before service and retain the recommendation documentation. Keep the actual evidence accessible to the reviewer. For example, when a clinic sends an unsigned request to schedule support, the agency still needs to obtain the required signed evidence before providing the service.
| Record | Recommended agency control | Question it answers |
|---|---|---|
| Recommendation | Store form or applicable standing order and dates | What supports this participant’s services? |
| Provider approval | Track individual NPI, pathway and enrollment | Who is approved to render care? |
| Prenatal support | Preserve time and cumulative units | How much has been delivered? |
| Birth support | Separate attendance from timed visits | Does the record support the birth service? |
| Postpartum support | Preserve time, stage and cumulative units | Which allowance applies? |
What enrollment and billing records should a group maintain?
The toolkit links Kansas enrollment and attestation instructions. Keep the qualification evidence requested for the selected application, individual provider approval and group arrangements distinct. A group’s administrative readiness should not substitute for each rendering worker’s eligibility.
Build an exception queue for missing recommendation evidence, unresolved provider approval, unsupported time and rejected payment requests. Assign an owner and next action to each issue. This is more actionable than one combined list of “incomplete cases.” Use the actual KanCare plan process for claims and corrections.
What should a doula retain for Kansas Medicaid revalidation?
Follow the actual KMAP revalidation notice for its evidence, dates and submission instructions. Keep the request, supporting records, submitted version and final decision together. A private credential's renewal cycle should not be substituted for a Medicaid revalidation notice.
A workforce calendar can track course or credential dates, Medicaid approval, revalidation requests and plan affiliation separately. Updating the agency's calendar does not establish that KMAP has accepted a submission.
Frequently asked questions
How should I check a training provider’s qualification claim?
Use the qualification routes in the current KMAP enrollment instructions. Ask the enrollment team to confirm acceptance of the exact credential or experience record.
How should course and experience evidence be organized?
Use the current application checklist for any required practical experience and attestation evidence. A course certificate is not itself a provider enrollment decision.
When should recommendation evidence be obtained?
KDHE directs doulas to obtain the signed evidence before service and retain it.
Are units the same as visits?
A visit and a payment unit describe different things. Apply the current payer definition to actual documented support.
Can birth attendance be telehealth?
The approved amendment does not allow telehealth for labor and delivery support.
Does a second doula create another allowance?
No. Reconcile the participant’s episode across workers.
How should support across pregnancies be recorded?
Keep each pregnancy and its actual services identifiable. Confirm the applicable frequency and documentation instructions before submitting another birth service.
Where should rates be checked?
Use the current KMAP fee schedule and applicable plan agreement for the service date.
